围术期联合应用复合肠道微生态制剂和谷氨酰胺对结直肠癌患者术后康复的影响
Effect of perioperative application of compound intestinal microecological agents combined with glutamine on postoperative recovery in patients with colorectal cancer
【摘要】 目的观察围术期联合应用肠道微生态制剂和谷氨酰胺对结直肠癌患者术后康复的影响。方法将127例结直癌患者随机分为试验组62例和对照组65例。对照组患者给予常规术前肠道准备和术后常规对症治疗,试验组患者术前给予口服肠道微生态制剂(双歧杆菌四联活菌片、乳果糖)和谷氨酰胺,术后在常规对症治疗的基础上继续口服双歧杆菌四联活菌片和谷氨酰胺。比较两组患者术前及术后第1、3、7天的血清C反应蛋白(CRP)和白细胞介素6(IL-6)水平,以及术后肛门排气时间、开始进食时间、术后住院天数、住院总费用、术后腹泻腹胀发生率。结果两组患者术后的血清CRP、IL-6水平均呈下降趋势,且试验组术后各时点的血清CRP、IL-6水平低于对照组(均P<0.05)。试验组术后肛门排气时间、进食时间均短于对照组,术后住院天数、住院费用均少于对照组,腹泻腹胀发生率低于对照组(均P<0.05)。结论围术期联合应用复合肠道微生态制剂与谷氨酰胺能有效地加速结直肠癌术后患者康复,降低患者炎症反应水平,节省患者住院费用。
【Abstract】 Objective To observe the effect of perioperative application of intestinal microecological agents combined with glutamine on postoperative recovery in patients with colorectal cancer. Methods Totally 127 patients with colorectal cancer were randomly divided into experimental group(n=62) and control group(n=65). Patients in the control group received conventional intestinal preparation before operation and routine symptomatic treatment after operation, patients in the experimental group were orally administrated intestinal microecological agents(Bifidobacterium Tetragenous Live Probiotics Tablets and lactulose) and glutamine before operation, and continued to orally take Bifidobacterium Tetragenous Live Probiotics Tablets and glutamine on the basis of routine symptomatic treatment after operation. Multiple indexes were compared between the two groups, including serum C-reactive protein(CRP) and interleukin 6(IL-6) levels before operation and on the 1 st, 3 rd and 7 th day after operation, duration for postoperative anal exhaust, duration for postoperative feeding, postoperative hospital stay, total hospitalization expenses, incidence rates of postoperative diarrhea and abdominal distension.Results After operation, serum CRP and IL-6 levels tended to decline in both groups, and the experimental group exhibited lower serum CRP and IL-6 levels at all time points after operation than the control group(all P<0.05). By comparison with the control group, the experimental group yielded shorter durations for postoperative anal exhaust and postoperative feeding, less postoperative hospital stay and total hospitalization expenses, and lower incidence rates of diarrhea and abdominal distension(all P<0.05).Conclusion Perioperative application of compound intestinal microecological agents combined with glutamine can effectively accelerate the postoperative recovery of patients with colorectal cancer, and reduce inflammatory response level and hospitalization costs of the patients.
