长期使用低剂量吸入性糖皮质激素对轻度哮喘患儿身高与肺功能的影响

【摘要】 目的探讨长期使用低剂量吸入性糖皮质激素对轻度哮喘患儿身高与肺功能的影响。方法纳入100例轻度哮喘患儿(哮喘组)和80例健康儿童(对照组)。哮喘组患儿入组后均规范使用低剂量吸入性糖皮质激素进行治疗。比较两组儿童入组时及入组后6个月、1年、2年的身高以及1年、2年身高增长速率。观察两组儿童入组时治疗前肺功能差异,以及哮喘组患儿治疗前及治疗后6个月、1年、2年的肺功能变化。结果两组儿童的身高均有随时间延长而增高的趋势(P<0.05),但两组的身高、1年和2年身高增长速率差异均无统计学意义(均P>0.05)。哮喘组治疗前的1秒用力呼气量(FEV1)和FEV1/用力肺活量的比值均低于对照组,但治疗后均有随治疗时间延长而升高的趋势(均P<0.05)。结论长期使用低剂量吸入性糖皮质激素治疗对轻度哮喘患儿的身高增长无显著影响,且能有效地改善患儿的肺功能。

【Abstract】 Objective To investigate the effect of long-term use of low-dose inhaled glucocorticoids(ICS) on height and lung function in children with mild asthma.Methods One hundred children with mild asthma(asthma group) and 80 healthy children(control group) were enrolled. Children in the asthma group were treated regularly with low-dose ICS after enrollment. The height on enrollment and six months, one year and two years after enrollment, and the 1-and 2-year growth rate of height were compared between the two groups. An observation was conducted on the differences between lung function on enrollment and before treatment in the two groups, as well as the changes in the lung function of the asthma group before treatment and six months, one year and two years after treatment. Results The height of children in both groups tended to increase with time(P<0.05), whereas there were no statistically significant differences in height, or 1-or 2-year growth rate of height between the two groups(all P>0.05). The forced expiratory volume in one second(FEV1) and FEV1/forced vital capacity ratio of the asthma group were lower than those of the control group before treatment, but tended to increase with the therapeutic time after treatment(all P<0.05).Conclusion Long-term use of low-dose ICS has no significant effect on the height growth of mild asthmatic children, and can effectively improve the lung function of the children.
