加温湿化高流量鼻导管氧疗在肺挫伤患者中的疗效研究

【摘要】 目的探讨加温湿化高流量鼻导管氧疗(HFNC)在肺挫伤患者中的应用效果。方法选取2017年7月至2019年6月茂名市人民医院外科收治的64例肺挫伤患者作为研究对象,按照随机数字表法分为观察组(n=32,给予HFNC)和对照组(n=32,给予常规面罩吸氧)。比较两组临床各项指标、治疗前后氧合指数、二氧化碳分压(PaCO2)和呼吸频率。结果观察组吸氧舒适度高于对照组,ICU住院时间短于对照组,有创机械通气率和28 d内病死率低于对照组(P <0.05)。治疗24 h后,观察组氧合指数高于对照组,PaCO2和呼吸频率低于对照组(P <0.05)。结论 HFNC用于肺挫伤患者效果确切,能明显提高患者吸氧舒适度,降低有创机械通气率和病死率,改善血气指标和呼吸节律,促进患者康复。

【Abstract】 Objective To investigate the effect of high-flow nasal catheter(HFNC) in patients with pulmonary contusion. Methods A total of 64 patients with pulmonary contusion admitted to the Department of Surgery of Maoming People’s Hospital from July 2017 to June 2019 were selected as the study subjects. They were divided into the observation group(n=32, given HFNC) and the control group(n=32, given conventional oxygen mask) according to the random number table method. The clinical indices, oxygenation index before and after treatment, partial pressure of carbon dioxide(PaCO2) and respiratory rate were compared between the two groups. Results The oxygen inhalation comfort of the observation group was higher than that of the control group, length of stay in intensive care unit(ICU) was shorter than that of the control group, and the rate of invasive mechanical ventilation and case fatality rate within 28 d were lower than those of the control group(P < 0.05). After 24 h of treatment, the oxygenation index in the observation group was higher than that in the control group, while PaCO2 and respiratory rate were lower than those in the control group(P < 0.05). Conclusion HFNC is effective in patients with pulmonary contusion, which can significantly improve the oxygen inhalation comfort, reduce the rate of invasive mechanical ventilation and case fatality rate, and improve the blood gas index and respiratory rhythm,and promote patient recovery.
