【摘要】 目的探讨肺炎克雷伯菌血流感染老年患者的临床特点。方法收集2016年1月～2018年9月吉林大学第一医院肺炎克雷伯菌血流感染的患者529例,分为老年组与非老年组,比较两组性别、住院时间、血中检出肺炎克雷伯菌前住院天数、感染部位、科室分布、基础疾病、易感因素、临床表现特点、疾病转归等。结果老年组入住ICU、插管(尿管、胃管、深静脉置管)、气管插管/机械通气、放疗/化疗、合并恶性肿瘤、脑血管疾病比例均高于非老年组,差异均具有统计学意义(P <0.05)。两组患者预后比较,差异无统计学意义(P> 0.05)。结论老年患者尽量减少不必要的侵入性操作;应重视合并恶性肿瘤、脑血管疾病的老年患者发生肺炎克雷伯菌感染。更多还原

【Abstract】 Objective To investigate the clinical characteristics of bloodstream infection caused by Klebsiella pneumoniae in older patients. Methods 529 patients with Klebsiella pneumoniae bloodstream infection in the First Hospital of Jilin University were enrolled from January 2016 to September 2018. The patients were divided into elderly group and non-elderly group. Sex,hospitalization days,days in hospital before detection of Klebsiella pneumoniae in blood,infection site,department distribution,basic diseases,susceptible factors,clinical manifestations,and prognosis were compared between the two groups. Results The proportions of ICU stay,intubation( urinary catheter,gastric catheter,deep vein catheter),tracheal intubation/mechanical ventilation,radiotherapy/chemotherapy,malignant tumor and cerebrovascular disease were significantly higher in elderly group than those in non-elderly group( P < 0. 05). There was no significant difference in the prognosis between the two groups( P > 0. 05). Conclusion Unnecessary invasive operations should be minimized in older patients,and attention should be payed to Klebsiella pneumoniae infection in older patients with malignant tumor and cerebrovascular disease.更多还原
