基于平乐正骨之推拿手法治疗神经根型颈椎病效果观察
Study on Massage Manipulation in Treating Cervical Spondylotic Radiculopathy Based on Pingle Bone Setting
【摘要】 目的:探究基于平乐正骨"筋滞骨错"平衡理论指导的推拿正骨手法治疗神经根型颈椎病的临床疗效。方法:选取神经根型颈椎病患者108例,按随机数表法分观察组和对照组,每组54例。对照组给予中药熏蒸+中成药内服治疗,观察组在对照组基础上给予基于平乐正骨"筋滞骨错"平衡理论指导的推拿正骨手法治疗,均治疗2个疗程。比较两组疼痛评分(visual analogue score,VAS)、颈椎功能评分(clinical assessment scale for cervical spondylosis,CASCS)、血液流变学指标[全血低切黏度(whole blood low shear viscosity,WBLSV)、全血高切黏度(whole blood high shear viscosity,WBHSV)]水平、临床疗效。结果:治疗后,观察组VAS评分、WBLSV、WBHSV水平低于对照组,CASCS评分中的症状、体征、工作和生活能力、手功能评分均高于对照组,差异有统计学意义(P<0.05)。总有效率观察组为98.15%(53/54),高于对照组的83.33%(45/54)(P<0.05)。结论:基于平乐正骨"筋滞骨错"平衡理论指导的推拿正骨手法治疗神经根型颈椎病可显著缓解患者疼痛症状,改善颈椎功能,提高治疗效果,其机制可能与改善血液流变学指标有关。
【Abstract】 Objective: To explore clinical effects of massage and bone setting manipulation in the treatment of cervical spondylotic radiculopathy based on Pingle bone setting "tendon stagnation and bone dislocation" balance theory. Methods: All 108 patients were allocated to the observation group and the control group in light of random number table method, 54 cases in each group. The control group was given herbal fumigation and oral administration of Chinese patent drugs, and the observation group massage and bone setting manipulation based on Pingle bone setting "tendon stagnation and bone dislocation" balance theory, both groups were treated for two courses. To compare VAS and CASCS between both groups, to detect the levels of WBLSV and WBHSV, and clinical effects. Results: After treating, VAS scores and the levels of WBLSV and WBHSV of the observation group were lower than these of the control group, the scores of the symptoms, body signs, ability to work and live,hand function in CASCS scores were higher than these of the control group, and the difference had statistical meaning(P<0.05). Total effective rate of the observation group was 98.15%(53/54), higher than 83.33%(45/54)of the control group(P<0.05). Conclusion: Massage and bone setting manipulation in the treatment of cervical spondylotic radiculopathy based on Pingle bone setting "tendon stagnation and bone dislocation" balance theory could notably relieve pain, improve cervical function, and raise therapeutic effects, and its mechanism might be related to improving hemorheology.
