热敏灸抗凋亡减轻脑缺血再灌注损伤

【摘要】 目的:基于大鼠热敏模型观察热敏灸对脑缺血再灌注损伤大鼠神经功能缺损、脑梗死体积变化及神经细胞凋亡的影响,探讨热敏灸治疗缺血性脑损伤的作用机理。方法:80只大鼠随机分为假手术组(20只)、缺血对照组(20只)及悬灸组(40只)。根据悬灸时大鼠尾温变化,悬灸组进一步分为非热敏灸组(7 d内尾温升高≤1℃)和热敏灸组(7 d内尾温升高>1℃)。应用大鼠改良神经功能缺损评分量表进行神经功能缺损评估,采用TTC染色法评估大鼠脑梗死体积,使用TUNEL试剂盒检测脑细胞凋亡情况。结果:与缺血对照组相比,热敏灸组大鼠缺血再灌注72 h及7 d的神经功能缺损评分均降低(P<0.05,P<0.01),而非热敏灸组与缺血对照组相比,于第7 d时降低了神经功能缺损评分(P<0.01)。脑缺血再灌注损伤72 h后热敏灸组大鼠脑梗死体积百分比低于非热敏灸组及缺血对照组(P<0.05,P<0.01)。大鼠脑组织切片TUNEL阳性染色光密度值在缺血再灌注后24 h,热敏灸组与缺血对照组及非热敏灸组相比均降低(两者均为P<0.05),而非热敏灸组与缺血对照组比较,差异无统计学意义(P>0.05);缺血再灌注后72 h,非热敏灸组与缺血对照组相比,光密度值降低(P<0.05),而热敏灸组与缺血对照组相比,光密度值降低(P<0.01),热敏灸组光密度值较非热敏灸组下降(P<0.05)。结论:热敏灸能有效地抑制脑缺血再灌注损伤大鼠神经细胞凋亡,减少脑梗死体积,改善大鼠神经功能。

【Abstract】 Objective:Based on the thermosensitive model,to observe the effect of heat-sensitive moxibustion on neurological deficit score,changes in cerebral infarction volume and neuronal apoptosis in rats with cerebral ischemia-reperfusion injury,and to explore the mechanism of heat-sensitive moxibustion in treating cerebral ischemia-reperfusion injury.Methods:A total of 80 rats were randomly divided into sham operation group(20 rats),ischemic control group(20 rats)and suspension moxibustion group(40 rats). According to the tail temperature changes of rats during suspension moxibustion,the suspension moxibustion group was further divided into non-heat-sensitive moxibustion group(tail temperature increased within 7 days ≤1℃)and heat-sensitive moxibustion group(tail temperature increased within 7 days >1℃). Modified neurological deficit scale was used to assess neurological deficit,infarct size was counted by TTC staining and TUNEL kit was used to detect apoptosis of brain cells.Results:Compared with ischemia control group,the neurological deficit score of heat-sensitive moxibustion group decreased significantly at 72 hours and 7 days after ischemia-reperfusion(P<0. 05,P<0. 01),while that of non-heat-sensitive moxibustion group decreased significantly at 7 days compared with ischemia control group(P<0. 01). The percentage of cerebral infarction volume in the thermosensitive moxibustion group was significantly lower than that in the non-thermosensitive moxibustion group and the ischemic control group after 72 hours of cerebral ischemia reperfusion injury(P<0. 05,P<0. 01). TUNEL positive staining light density of brain slices in rats decreased significantly 24 hours after ischemia-reperfusion compared with ischemia control group and non-heat-sensitive moxibustion group(both P<0. 05),while there was no significant difference between nonheat-sensitive moxibustion group and ischemia control group(P>0. 05);72 hours after ischemia-reperfusion,non-heatsensitive moxibustion group and non-heat-sensitive moxibustion group had no significant difference(P>0. 05). Compared with the ischemic control group,the optical density of the heat-sensitive moxibustion group decreased significantly(P<0. 01),and the optical density of the heat-sensitive moxibustion group decreased significantly(P<0. 05)compared with the non-heat-sensitive moxibustion group.Conclusion:Heat-sensitive moxibustion can effectively inhibit the apoptosis of nerve cells,reduce the volume of cerebral infarction,and improve the score of nerve function defect in rats with cerebral ischemia reperfusion injury.
