外固定架胫骨横向搬移微循环重建技术治疗下肢慢性缺血性疾病的临床观察

【摘要】 目的观察外固定架胫骨横向搬移微循环重建技术治疗下肢慢性缺血性疾病的作用。方法下肢慢性缺血性疾病42例患者随机分为治疗组与常规组,每组21例。治疗组采用外固定架胫骨横向搬移微循环重建技术治疗,常规组采用单纯介入治疗。比较两组患者术后足部皮温、疼痛视觉模拟评分(VAS)、踝肱指数(ABI)、患侧足背动脉流速及并发症。结果术后1个月,治疗组患者足部皮温、ABI明显高于常规组,足背动脉血流速度明显快于常规组,VAS评分明显低于常规组,差异均有统计学意义(P<0.01)。两组患者术后并发症发生率的差异无统计学意义(P>0.05)。结论经外固定胫骨横向搬移微循环重建技术治疗下肢慢性缺血疾病,症状缓解且预后良好,具有良好的临床应用价值。

【Abstract】 Objective To observe the effect of microcirculation reconstruction with transverse tibial bone transport with external fixator in the treatment of chronic ischemic diseases of lower limbs. Methods Forty-two patients with chronic ischemic diseases of lower extremities were randomly divided into the Treatment Group and Conventional Group, 21 cases in each group. The Treatment Group was treated with microcirculation reconstruction with transverse tibial bone transport with external fixator while the Conventional Group was treated with pure interventional treatment. The postoperative foot skin temperature, pain visual analogue scale(VAS), ankle-brachial index(ABI), flow velocity of the dorsal artery of the affected side and complications were compared between the two groups.Results The foot skin temperature and ABI in the Treatment Group were significantly higher than those in the Conventional Group at 1 month after surgery, the flow velocity of the dorsal artery of the foot in the Treatment Group was significantly faster than those in the Conventional Group, and the VAS score in the Treatment Group was lower than that of the Conventional Group, and the differences were statistically significant(P<0.01). There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion Microcirculation reconstruction with transverse tibial bone transport with external fixator can relieve the symptoms has good prognosis in the treatment of chronic ischemic diseases of the lower limbs. It has good clinical application value.
