【摘要】 目的观察N-乙酰半胱氨酸联合无创正压通气治疗慢性阻塞性肺疾病（COPD）合并Ⅱ型呼吸衰竭的疗效。方法 90例COPD合并Ⅱ型呼吸衰竭患者给予无创正压通气及常规治疗2周,联合组增加N-乙酰半胱氨酸治疗。比较两组治疗效果、住院时间、住院费用及肺功能指标、血气指标、生活质量情况。结果与对照组比较,联合组疗效较好,FEV1、PEF、FEV1预计值、PaO₂、SaO₂升高,而住院时间与费用、PaCO₂、CAT评分较低（P<0.01或0.05）。结论 N-乙酰半胱氨酸联合无创正压通气治疗COPD合并Ⅱ型呼吸衰竭可改善肺功能与血气功能,提高生活质量。更多还原

【Abstract】 Objective To observe the efficacy of N-acetylcysteine and non-invasive positive pressure ventilation（NIPPV） in chronic obstructive pulmonary disease（COPD） with type Ⅱ respiratory failure. Methods Ninety patients with COPD and type Ⅱ respiratory failure were treated with NIPPV and conventional therapy for 2 weeks, and N-acetylcysteine was only added in combined group. Therapeutic efficacy, hospitalization time and cost, lung functio, blood gas, and quality of life were compared between two groups. Results Compared with control group, therapeutic efficacy, FEV1, PEF, FEV1,PaO₂, and SaO₂ were higher, while hospitalization time and cost, PaCO₂, and CAT scores were lower in combined group（P<0.01 or 0.05）. Conclusion Combined N-acetylcysteine and NIPPV can improve lung function, blood gas and quality of life in patients with COPD and type Ⅱ respiratory failure.更多还原
