白血病患者T淋巴细胞及免疫球蛋白检测价值评价

【摘要】 目的探讨T淋巴细胞及免疫球蛋白在白血病患者血液检测中的价值。方法选取2015年1月至2019年1月本院收治的45例初诊和12例治疗后缓解的急性白血病患者作为白血病组,另选取同期健康体检者60名作为健康组。两组患者均采用流式细胞仪,ELISE等方法进行静脉血检测,比较两组Th1及Th2细胞因子水平、CD3+,CD4+,CD8+及CD4+/CD8+占比和免疫球蛋白水平差异性。结果白血病组患者Th1细胞中IL-2、IFN-γ表达低于健康组,缓解时患者Th1细胞中IL-2、IFN-γ表达高于初诊时,白血病组Th2细胞中IL-4、IL-10水平高于健康组(P<0.05),缓解时患者Th2细胞中IL-4、IL-10表达低于初诊时(P<0.05);初诊时白血病患者免疫球蛋白IgA、IgM高于健康组,IgG低于健康组(P<0.05),缓解时白血病患者免疫球蛋白IgG、IgM水平与健康组比较差异无统计学意义,IgA高于健康组(P<0.05);白血病组患者在初诊时T淋巴细胞CD3+、CD4+、CD4+/CD8+低于健康组,CD8+高于健康组(P<0.05),与初诊时比较,缓解时白血病患者T淋巴细胞CD3+、CD4+、CD4+/CD8升高,CD8+下降(P<0.05);白血病组患者外周血CD4+CD25+、CD4+CD25+high高于健康组(P<0.05),缓解时白血病患者外周血CD4+CD25+、CD4+CD25+high低于初诊时(P<0.05);白血病患者初诊时乳酸托氢酶(LDH)水平高于健康组(P<0.05),缓解时LDH水平低于初诊时(P<0.05)。结论急性白血病患者T淋巴细胞细胞及免疫球蛋白表达异常,其指标变化在急性白血病患者的治疗指导具有临床意义。

【Abstract】 Objective To investigate the value of T lymphocytes and immunoglobulin in blood testing of leukemia patients. Methods Select 45 patients with first visit and 12 patients with acute leukemia who have remission after treatment who were admitted to our hospital from January 2015 to January 2019 were selected as the leukemia group, and another 60 healthy physical examination patients during the same period were selected as the healthy group. The levels of Th1 and Th2 cytokines, CD3+, CD4+, CD8+and the proportion of CD4+/CD8+and the difference of immunoglobulin levels between the two groups were compared by flow cytometry and ELISE. Results The expression of IL-2 and IFN-γ in Th1 cells of leukemia group was lower than that of healthy group. The expression of IL-2 and IFN-γ in Th1 cells of patients with remission were higher than those of patients with outpatient treatment. The levels of IL-4 and IL-10 in Th2 cells of leukemia group were higher than those of healthy group(P<0.05). The expression of IL-4 and IL-10 in Th2 cells of patients with remission were lower than those of patients with outpatient treatment(P<0.05). The immunoglobulin IgA and IgM of leukemia patients were higher than those of healthy group at the initial diagnosis, and IgG was lower than that of healthy group(P<0.05). There was no statistically significant difference in the levels of immunoglobulin IgG and IgM between the leukemia group and the healthy group in remission, and IgA was higher than that of the healthy group(P<0.05), CD3+, CD4+, CD4+/CD8+of T lymphocyte in leukemia group were lower than those in healthy group, CD8+was higher than that in healthy group(P<0.05), CD3+, CD4+, CD4+/CD8+of T lymphocyte increased and CD8+decreased in remission(P<0.05) while CD4+CD25+, CD4+CD25+of peripheral blood in leukemia group were higher than those in healthy group(P<0.05). The levels of CD4+CD25+, CD4+CD25+highin peripheral blood of patients at resolution were lower than those at initial diagnosis(P<0.05); The level of lactate hydrolase(LDH) in leukemia patients was higher than that in healthy group(P<0.05), and the level of LDH in remission was lower than that in the initial diagnosis(P<0.05). Conclusion The expression of T lymphocyte and immunoglobulin in patients with acute leukemia is abnormal, and its index changes have clinical significance in the treatment of acute leukemia patients.
