床旁超声监测胃残余量在脓毒症休克患者肠内营养中的应用
Application of bedside ultrasound to monitor gastric residual volume in enteral nutrition of patients with sepsis
【摘要】 目的探讨床旁超声检测胃残余量在脓毒症休克患者肠内营养中的应用效果。方法选取2018年4月至2019年7月本院收治的84例脓毒症行肠内营养支持患者作为研究对象,按随机数字表法分为对照组和观察组,每组42例。对照组采用胃管回抽法监测胃残余量,观察组采用床旁超声法监测胃残余量,比较两组胃残余量、营养状态和并发症情况。结果两组胃残余量比较差异无统计学意义;观察7 d后,两组总蛋白、白蛋白、淋巴细胞计数比较差异均无统计学意义;观察组肠内营养中断发生率为9.52%,呼吸机相关肺炎发生率为4.76%,低于对照组的33.33%、23.81%,差异均有统计学意义(P<0.05)。结论使用床旁超声监测脓毒症休克患者的胃残余量有助于提高测量准确性,并对喂养耐受性实现动态监测,降低并发症发生率,值得临床推广应用。
【Abstract】 Objective To investigate the effect of bedside ultrasound on gastric residual volume in enteral nutrition in patients with sepsis.Methods 84 patients with enteral nutrition support for sepsis admitted to our hospital from April 2018 to July 2019 were selected as the research subjects. The patients were divided into the control group and the observation group according to the random number table, with 42 cases in each group.The gastric residual volume was monitored by gastric tube extraction. The observation group was monitored by bedside ultrasound to monitor the residual amount of the stomach, and the gastric residual, nutritional status and complications were compared between the two groups. Results There was no significant differences in the residual amount of gastric juice between the two groups. After 7 days, there was no significant difference in the total protein, albumin and lymphocyte counts between two groups; the incidence of enteral nutrition interruption in the observation group was 9.52%,and the incidence of ventilator-associated pneumonia was 4.76% lower than that in the control group(33.33%, 23.81%), the differences was statistically significant(P<0.05). Conclusions Bedside ultrasound monitoring of gastric residuals in patients with sepsis can improve measurement accuracy and achieve dynamic monitoring of feeding tolerance and reduce the incience of complications, worthy of clinical promotion and application.
