氨甲环酸治疗初次单侧全膝关节置换术后出血疗效的Meta分析

【摘要】 目的:比较氨甲环酸局部应用与静脉应用在初次单侧全膝关节置换术中的疗效。方法:通过检索PubMed、Cochrane Library、Embase、Web of Science英文数据库获取文献,采用Cochrane推荐的风险偏倚评估工具进行质量评价,采用Revman 5.3软件和Stata 12.0软件对效应指标进行统计分析。结果:共计28篇文献,33组随机对照实验,患者共计3 423人,实验组1 673人,对照组1 750人,2组总出血量(MD=-24.41,95%CI=-63.12~14.30)、引流量(MD=-5.24,95%CI=-42.86~32.38)、血红蛋白下降值(MD=1.73,95%CI=-0.31~3.79)、输血率(MD=1.12,95%CI=0.89~1.42)、血栓相关并发症(MD=0.98,95%CI=0.58~1.65)比较差异均无统计学意义。结论:初次单侧全膝关节置换术中氨甲环酸局部与静脉两种给药方式疗效无差异。

【Abstract】 Objective: To compare the efficacy of local application and intravenous application of tranexamic acid in the initial unilateral total knee arthroplasty. Methods: Through searching PubMed, Cochrane Library, Embase and Web of Science to obtain literature, the risk bias assessment tool recommended by Cochrane was used for quality evaluation, and the Revman 5.3 software and Stata 12.0 software were used for statistical analysis of the effect indicators. Results: A total of 28 pieces of literature, 33 groups of randomized controlled trials were included, and there were 3 423 patients, 1 673 in the experimental group, 1 750 in the control group. There were no statistically significant differences in total bleeding(MD=-24.41, 95%CI=-63.12~14.30), drainage volume(MD=-5.24, 95%CI=-42.86~32.38), hemoglobin decrease value(MD=1.73, 95%CI=-0.31~3.79), blood transfusion rate(MD=1.12, 95%CI=0.89~1.42), thrombosis-related complications(MD=0.98, 95%CI=0.58~1.65). Conclusion: In the initial unilateral total knee arthroplasty, the intravenous and intra-articular application of tranexamic acid has similar effectiveness and safety.
