慢性脑供血不足患者三氧自血疗法治疗前后SPECT/CT脑血流灌注显像及MRI影像变化分析

【摘要】 目的分析慢性脑供血不足(CCCI)患者三氧自血疗法治疗前后单光子发射计算机断层摄影(SPECT)/CT脑血流灌注显像及MRI影像变化。方法选取2018年6月至2020年6月我院收治的CCCI患者60例为研究对象,均经三氧自血疗法治疗1个疗程,比较其治疗前后SPECT/CT参数[局部脑血流量(rCBF)、靶/非靶值(T/NT)、标准化摄取值(SUV)]及MRI灌注加权成像(PWI)影像学参数[脑血流量(CBF)、局部脑血容量(rCBV)、平均通过时间(MTT)、最大峰值时间(TTP)]变化,分析影像学参数与认知功能[简易精神状态量表(MMSE)、蒙特利尔认知评估量表(MoCA)]的相关性。结果与治疗前比较,治疗后CCCI患者SPECT/CT中rCBF、T/NT、SUV值均增加,MRI.-PWI的CBF、rCBV增加,而MTT、TTP缩短(P<005);治.疗后60例患者MMSE评分、MoCA评分均增加(P<005);相关分析显示,CCCI患者SPECT/CT中rCBF、T/NT、SUV及PWI中CBF、rCBV与MMSE评分、MoCA评分呈正相关,SPECT/CT中T/NT、SUV及PWI中.MTT、TTP与MMSE评分、MoCA评分呈负相关(P<005)。结论CCCI患者经三氧自血疗法治疗后SPECT/CT脑血流灌注显像及MRI影像发生明显变化,且其与患者认知功能有关,进行影像学监测有利于实现CCCI的早期诊疗。

【Abstract】 Objective to analyze the image changes of single-photon emission computed tomography(SPECt)/Ct cerebral blood perfusion imaging and MRI before and after ozone autohemotherapy for patients with chronic cerebral circulation insufficiency(CCCI). Methods A total of 60 patients with CCCI who were treated in the hospital from June 2018 to June 2020 were selected as the research subjects.All of them were treated with ozone autohemotherapy for a course. SPECt/Ct parameters [regional cerebral blood flow(rCBF), target/non-target(t/Nt), standardized uptake value(SUV)] and imaging parameters of MRI perfusion-weighted imaging(PWI) [cerebral blood flow(CBF), regional cerebral blood volume(rCBV), mean transit time(Mtt), time-to-peak(ttP)] were compared before and after treatment. the correlation between imaging parameters and cognitive function [mini-mental state examination(MMSE), Montreal Cognitive Assessment(MoCA)] was analyzed. Results For patients with CCCI after treatment, rCBF, t/Nt, and SUV in SPECt/Ct were increased. CBF and rCBV in MRI-PWI were increased, while Mtt and ttP were shortened(P<0.05). After treatment, the MMSE and MoCA scores of all subjects were increased(P<0.05). Correlation analysis showed that rCBF, t/Nt, and SUV in SPECt/Ct, CBF and rCBV in PWI were positively correlated with the MMSE and MoCA scores of patients with CCCI, while Mtt and ttP in PWI were negatively correlated with the scores(P<0.05). Conclusion there are significant image changes in SPECt/Ct cerebral blood perfusion imaging and MRI of patients with CCCI after ozone autohemotherapy, related to their cognitive function. Imaging monitoring is conducive to the early diagnosis and treatment of CCCI.
