动态增强MRI、超声与钼靶对乳腺良、恶性病灶的诊断价值对比
Comparison on the Diagnostic Value of Dynamic Enhanced MRI, Ultrasound, and Molybdenum Target for Benign and Malignant Lesions of the Breast
【摘要】 目的分析动态增强MRI、超声与钼靶对乳腺良、恶性病灶的诊断价值对比。方法回顾分析本院2017年6月至2019年6月收治的经病理组织验证的70例乳房肿瘤患者的临床资料,所有病例均进行了动态增强MRI、超声与钼靶检查,观察三种检查的影像学表现及病理组织结果,对三种检查对乳房良、恶性病灶诊断的准确性、敏感性、特异性结果进行分析。结果超声和钼靶检查对乳腺肿瘤的病灶大小均明显小于术后病理检测病灶大小(P<0.05);而动态增强MRI检查的乳腺肿瘤病灶大小与术后病理检测病灶大小比较未见明显差异(P>0.05)。三种单一检查中,动态增强MRI的灵敏性和准确性较高,分别达到了84.21%、77.14%,而钼靶的特异性最高,为93.75%;超声+钼靶+动态增强MRI联合检查诊断乳腺肿瘤灵敏性、准确性均高于单重单一检查(P<0.05)。结论增强MRI较单一的超声、钼靶检查对乳腺良、恶性病灶的诊断灵敏性、准确性更高,检测病灶大小效能更好,但特异性较低,可有效联合检查提高对乳腺肿瘤良性、恶性病灶的确诊率。
【Abstract】 Objective To analyze the diagnostic value of dynamic enhanced MRI,ultrasound,and molybdenum targets for benign and malignant breast lesions.Methods The clinical data of 70 patients with breast tumors confirmed by pathological tissue treated in our hospital from June 2017 to June 2019 were retrospectively analyzed.All cases were performed with dynamic enhanced MRI,ultrasound,and molybdenum target.The imaging findings and pathological results of the three examinations were observed,and the accuracy,sensitivity,and specificity of the three tests in the diagnosis of benign and malignant lesions of the breast were analyzed.Results The lesion size of breast tumors detected by ultrasound and molybdenum target was significantly smaller than that by postoperative pathological exa mination(P<0.05).There was no significa nt difference in the lesion size of breast tumors between the dynamic enhanced MRI and the pathological examination(P>0.05).Among the three single examinations,the sensitivity and accuracy of dynamic enhanced MRI were higher,reaching 84.21%and 77.14%,respectively,while the specificity of the molybdenum target was the highest,reaching93.75%.Fu rthermore,sensitivity and accura cy of ult rasound+m olybdenum target+dynamic enhanced MRI in the diagnosis of breast tumor were higher than that of a single examination(P<0.05).Conclusion Compared with single ultrasound and molybdenum target,enhanced MRI has higher accuracy and sensitivity in diagnosing benign and malignant lesions of the breast.The efficacy of detecting the size of the lesion is better,but the specificity is lower.Therefore,it ca n be effectively com bined to improve breast tumors’ diagnosis for benign and malignant lesions.
