高血压早期肾损伤MSCT灌注成像表现及其与尿微量白蛋白的相关性

【摘要】 目的观察高血压早期肾损伤多层螺旋CT(MSCT)灌注成像表现,分析其与尿微量白蛋白(m-Alb)的相关性。方法选取我院收治的96例原发性高血压肾损伤患者纳入观察组,并取同期96名健康体检者纳入对照组,均行MSCT灌注成像检查、尿常规与肾功能检查,比较2组一般资料、肾皮质与肾髓质灌注参数[血流量(BF)、血容量(BV)、毛细血管通透性(PS)、平均通过时间(MTT)],并分析MSCT灌注参数与m-Alb的相关性。结果 2组性别、年龄、BMI、空腹血糖及肾功能指标水平比较无明显差异(P>0.05);观察组m-Alb水平明显高于对照组(P<0.05);观察组肾皮质与肾髓质BF、BV、PS均明显小于对照组(P<0.05),MTT明显长于对照组(P<0.05);高血压早期肾损伤患者肾皮质与肾髓质BF、BV、PS与患者m-Alb呈负相关(P<0.05),MTT与患者m-Alb呈正相关(P<0.05)。结论高血压早期肾损伤患者MSCT灌注成像中肾皮质与肾髓质BF、BV、PS与m-Alb呈负相关,MTT与m-Alb呈正相关,在高血压早期肾损伤临床诊断中具有较高应用价值。

【Abstract】 Objective To observe the representation of multi-slice CT(MSCT) perfusion imaging for hypertension and early renal injury,to analyze its correlation with microalbuminuria(m-Alb).Methods 96 patients with primary hypertension and renal injury admitted to the hospital were enrolled in the observation group.96 healthy subjects in the same period were enrolled in the control group.All patients underwent examination of MSCT perfusion imaging,urine routine and renal function.The general data,perfusion parameters of renal cortex and medulla [blood flow(BF),blood volume(BV),permeability surface(PS),mean transit time(MTT))] of the two groups were compared.The correlation between MSCT perfusion parameters and m-Alb was analyzed.Results There were no significant differences in gender,age,BMI,fasting blood glucose,and level of renal function index between the two groups(P>0.05).The m-Alb level in the observation group was significantly higher than that in the control group(P<0.05).The BF,BV,and PS of the renal cortex and medulla in the observation group were significantly lower than those in the control group(P<0.05),while MTT was significantly longer than that in the control group(P<0.05).BF,BV and PS of the renal cortex and medulla in patients with hypertension and early renal injury were negatively correlated with m-Alb of patients(P<0.05).And MTT was positively correlated with m-Alb(P<0.05).Conclusion BF,BV,and PS of the renal cortex and medulla in MSCT perfusion imaging of patients with hypertension and ea rly renal injury are negatively correlated with m-Alb.MTT is positively correlated with m-Alb,which has higher application value in the clinical diagnosis of hypertension and ea rly renal inju ry.
