【摘要】 目的探讨钆塞酸二钠(Gd-EOB-DTPA)增强MRI对肝细胞肝癌(HCC)的诊断及其分化程度的预测价值。方法回顾性分析2017年1月至2019年1月我院确诊并收治的82例行肝功能检查患者的临床资料,将所有患者按检查方法不同分为研究组(n=48)和对照组(n=34),研究组行Gd-EOB-DTPA增强MRI检查,对照组行常规MRI检查。以美国肝脏病研究协会标准为诊断"金标准",比较两组MRI图像特点及病灶检出率;定量分析Gd-EOB-DTPA增强MRI检查的信号对比增强率,并分析其与病理学检查一致性;行ROC曲线比较两组诊断HCC的敏感度、特异性。结果两组图像中边缘规整度、信号强度有统计学差异(P<0.05);研究组对病灶检出率特别是对1~1.5cm病灶检出率高于对照组(P<0.05);病理学分级与对比增强率呈显著负相关关系(P<0.05);Gd-EOB-DTPA增强MRI检查诊断HCC的诊断效能优于常规MRI检查(P<0.05)。结论 Gd-EOB-DTPA增强MRI检查是诊断HCC有效方法,能提高诊断准确性,且对HCC病理学分级诊断价值较高,可在临床广泛推广应用。

【Abstract】 Objective To investigate value of gadolinium-ethoxy benzyl diethylenet riaminepentaacetic acid(Gd-EOBDTPA) enhanced M RI in the diagnosis and differentiation of hepatocellular carcinoma(HCC).Methoods The clinical data of 82 HCC patients admitted to our hospital between January 2017 and January2019 were retrospectively analyzed.All patients were divided into study group(n=48) and control group(n=34) according to different examination methods.The study group underwent Gd-EOB-DTPA enhanced MRI,and the control group underwent routine MRI.Taking American Association for the Study of Liver Diseases(AASLD) criteria as the diagnostic gold standa rd,cha racteristics of M RI images and the detection rate of lesions were compared between the two groups.The lesion signal contrast ratio of Gd-EOB-DTPA enhanced MRI was quantitatively analyzed,and the consistency between GdEOB-DTPA enhanced MR imaging and pathological examination was also determined.Results The edge regularity and signal intensity of the images had significant difference between two groups(P<0.05).The detection rate of 1-1.5 cm lesions in the study group was higher than that in the control group(P<0.05).Pathological grade was negatively correlated with contrast enhancement rate(P<0.05).The diagnostic efficiency of Gd-EOB-DTPA enhanced MRI for HCC was better than that of conventional MRI(P<0.05).Conclusion Gd-EOB-DTPA enhanced MRI is an effective method for diagnosing HCC,which can improve the diagnostic accuracy,and has a high diagnostic value for HCC pathological grade,so it is worthy of promotion.
