神经内镜与显微镜手术治疗无功能性垂体腺瘤疗效对比

【摘要】 目的:观察神经内镜与显微镜经鼻蝶切除无功能性垂体腺瘤的临床效果。方法:2017年1月~2020年1月收治的116例无功能性垂体腺瘤(NFPA)患者为研究对象,随机分为观察组(神经内镜下经鼻蝶切除术)与对照组(显微镜下经鼻蝶切除术),比较两组患者的治疗效果、并发症发生率及肿瘤复发率。结果:与对照组相比,观察组患者的手术时间更短(P<0.05)、术中出血量更少(P<0.05);两组患者的肿瘤切除程度、临床症状缓解情况、并发症发生率及肿瘤复发率比较,差异均无统计学意义(P>0.05)。结论:神经内镜下经鼻蝶切除无功能性垂体腺瘤能缩短手术时间,减少术中出血量,值得推荐为NFPA的首选手术方法,但术后远期并发症情况仍需进一步随访观察。

【Abstract】 Objective:To explore the clinical effect of neuroendoscopic and microscopic nasal sphenoid resection of non-functional pituitary adenomas. Methods:A total of 116 patients with non-functional pituitary adenoma(NFPA) admitted to our hospital from January 2017 to January 2020 were selected as the research objects. According to the random number table method, they were divided into the study group(neuroendoscopic transsphenoidal resection) and the control group(transnassphenoidal resection under the microscope). The treatment effect, complication rate and tumor recurrence rate between the two groups were compared. Results:The study group had shorter operation time and less intraoperative blood loss, and the difference was statistically significant(P<0.05); the difference in the degree of tumor resection, clinical symptom relief, complication rate and tumor recurrence rate between the two groups.There was no statistical significance(P>0.05). Conclusion:Neuroendoscopic transnasal resection of non-functional pituitary adenomas has a good clinical effect and high safety. This method is worth recommending as the preferred surgical method for NFPA.
