【摘要】 目的:分析sICAM-1和IL-36在儿童肺炎支原体肺炎(mycoplasma pneumoniae pneumonia, MPP)中的表达水平及两者在肺炎支原体肺炎中的临床意义。方法:根据第八版《诸福棠实用儿科学》社区获得性肺炎(community-acquired pneumonia, CAP)的诊断标准,选取2019年3月至2020年1月于内蒙古包头市第四医院儿科确诊为肺炎的患儿66例,根据MPP的诊断标准分为MPP组(28例),非MPP对照组(38例),选取同期支气管异物儿童16例为正常对照组,采用ELISA法测定肺泡灌洗液中可溶性细胞间粘附分子-1(soluble intercellular adhesion molecule-1,sICAM-1)和白介素(interleukin-36,IL-36)的水平,用ROC曲线评价IL-36、sICAM-1对MPP的诊断效能。结果:MPP组sICAM-1和IL-36表达水平高于非MPP组(P<0.05)及正常对照组(P<0.05)。非MPP组sICAM-1和IL-36表达水平高于正常对照组(P<0.05),sICAM-1诊断肺炎支原体肺炎的AUC为0.818(95%可信区间为0.703-0.902),其敏感度为71.43%,特异度为84.21%;IL-36对诊断MPP的AUC为0.875(95%可信区间为0.770-0.944),其敏感度为75.00%,特异度为89.47%。结论:检测sICAM-1和IL-36在MPP中表达水平有助于MPP患儿的临床诊断。更多还原

【Abstract】 Objective:To analyze the levels of expression and clinical significance of sICAM-1 and IL-36 among children with mycoplasma pneumoniae pneumonia(MPP). Methods:According to the diagnostic criteria of MPP and CAP in the eighth edition of Zhufutang Practical Pediatrics, a total of 66 children with pneumonia in paediatric department of Baotou Fourth Hospital were selected and divided into MPP group(28 children) and non-MPP group(38 children). Sixteen children with bronchial foreign bodies in the same period were selected as the normal control group. ELISA was used to detect the levels of sICAM-1 and IL-36 in alveolar lavage fluid(BALF), and the receiver operating characteristic(ROC) curve was used to evaluate the diagnosis efficiency for MPP. Results:sICAM-1 and IL-36 expression levels in MPP group were higher than those in non-MPP group(P<0.05) and normal control group(P<0.05). sICAM-1 and IL-36 expression levels in the non-MPP group were higher than those in the normal control group(P<0.05). The area under ROC curve of sICAM-1 was 0.818(95 % CI 0.703-0.902), and the sensitivity and specificity of the sICAM-1 were 71.43 % and 84.21 %. The area under ROC curve of IL-36 was 0.875(95 % CI 0.770-0.944), and the sensitivity and specificity of the IL-36 were 75.00 % and 89.47 %. Conclusion:The detection of sICAM-1 and IL-36 expression levels in MPP is helpful for the clinical diagnosis of children with MPP.更多还原
