【摘要】 目的:探讨分析电子输尿管软镜钬激光碎石术(RIRS)治疗上尿路结石的疗效。方法:选取2018年4月至2020年10月收治的398例上尿路结石患者分为观察组和对照组,观察组给予电子输尿管软镜钬激光碎石术治疗,对照组给予经皮肾镜穿刺碎石术治疗,比较两组结石清除率、手术指标、术后恢复指标、并发症。结果:观察组结石清除率近期为89.95%(179/199)、远期为96.98%(193/199),与对照组的90.95%(181/199)和95.98%(191/199)同期对比相当,差异无统计学意义(χ~2=0.116,0.296,P=0.733,0.586);观察组手术时间较对照组显著延长,术中出血量较对照组显著减少,术后疼痛发生率为32.66%较对照组100.00%显著降低,术后下床时间及住院时间较对照组显著缩短,术后并发症发生率为14.07%较对照组24.62%显著降低(P<0.05)。结论:RIRS治疗上尿路结石效果显著,可缩短住院时间,减少术中出血量,降低疼痛率和并发症发生率,促进患者恢复。更多还原

【Abstract】 Objective:To explore and analyze the efficacy of flexible ureteroscope holmium laser lithotripsy in the treatment of upper urinary calculi. Methods:398 patients with upper urinary calculi admitted from April 2018 to October 2020 were chosen and divided into the observation group and the control group. The observation group were treated with flexible electronic ureteroscope holmium laser lithotripsy, while the control group were treated with percutaneous nephrolithotomy, with the stone removal rate, the surgical index, the postoperative recovery index and complications compared. Results:The stone removal rate of the observation group was 89.95 %(179/199) in the short term and 96.98 %(193/199) in the long term, similar to 90.95 %(181/199) and 95.98 %(191/199) in the control group during the same period, the difference being not statistically significant(χ~2=0.116,0.296, P=0.733,0.586); the operation time in the observation group was longer than that of the control group; the intraoperative blood loss in the observation group was more significantly reduced than that of the control group; the incidence of postoperative pain in the observation group was 32.66 %, significantly lower than that of the the control group(100.00 %); the postoperative time to get out of bed and hospital stay in the observation group were significantly shorter than those of the control group; the postoperative complication rate in the observation group was 14.07 %, significantly lower than that of the control group(24.62 %)(P<0.05). Conclusion:Flexible ureteroscope holmium laser lithotripsy is very effective in treating upper urinary tract calculi, which can shorten hospital stay, reduce the blood loss during the operation, reduce the pain rate and the incidence of complications and promote the recovery of patients.更多还原
