某县域内全科医师毕业后医学教育现况和需求分析
Current situation and demand analysis of post-graduate medical education in a county
【摘要】 目的本研究旨在探究某县域内全科医师的毕业后医学教育现况和需求,为有效开展基层全科医师培训提供依据。方法 2020年1月以自行设计的基层全科医师毕业后医学教育现况和需求调查问卷,对参加浙江大学医学院附属邵逸夫医院培训的某县域基层全科医师进行调查。内容包括全科医师毕业后医学教育现况、门诊病种及培训需求。结果 114例基层全科医师中,52.6%为副主任医师及主治医师,47.4%为住院医师,41.3%未参加过全科培训。针对期望提升方向,副主任及主治医师在全科教学及科研(28.3%vs 3.7%,P<0.001)、全科医学人文(21.7%vs 1.9%,P=0.003)有更高的需求。不同级别医师均认为最有必要培训的技能是急救技能。结论针对培训方向,不同职级基层全科医师的需求存在明显差异;针对培训方式和带教方式,全科医师的需求较为多样化。建议对基层全科医师进行分级、分层培养,形成精准化、多元化、级联式的基层全科医师培养方法,为解决基层全科医师能力不足的问题、稳步推进分级诊疗制度提供新思路。
【Abstract】 Objective The purpose of this research was to explore the status and needs of post-graduate medical education of general practitioners in a county, and to provide a basis for the effective development of primary general practitioner training. Methods In January 2020, with a self-designed questionnaire on the current status and needs of post-graduate medical education, a survey was conducted on primary general practitioners who would participate in the training of general practitioners in Sir Run Run Shaw Hospital, Zhejiang University School of Medicine. The content included the general practitioner training status, clinic diseases and training needs. Results Among the 114 primary general practitioners, 52.6% were associate chief physicians and attending doctors, 47.4% were resident doctors, and 41.3% had not participated in post-graduate medical education. For the direction of expectation improvement, associate chief physicians and attending doctors had higher demands in general teaching and research(28.3 % vs 3.7 %,P<0.001) and general medical humanities(21.7% vs 1.9%, P=0.003). Doctors at different levels believed that the most necessary training skills were first-aid skills. Conclusion According to the needs of training direction, there are obvious differences among primay general practitioners of different ranks. In terms of training and teaching, the needs of general practitioners are diversified. It is suggested that post-graduate medical education of primary general practitioners should be stratified, and the higher level should focus on the cultivation of teaching ability, so as to form an accurate, diversified and progressive training mode.
