【摘要】 目的:验证旋提手法联合舒适牵引治疗椎动脉型颈椎病的临床价值。方法:116例椎动脉型颈椎病引起颈性眩晕的患者随机分组,试验组接受旋提手法+舒适机械牵引,对照组采用传统治疗方案为红外线照射+机械牵引,2日1次,7次为1个疗程。分析比较2种治疗方案的有效率、治疗前后患者颈部疼痛视觉模拟评分(VAS)、FS-CSA 量表眩晕疗效评价、椎动脉收缩期峰值血流速度、平均血流速度等血流参数、操作产生的不良反应等指标。结果:旋提手法+舒适机械牵引治疗的有效率大于红外线照射+机械牵引,差异具有统计学意义。临床疗效评价中,2组治疗后效果均强于治疗前。在VAS评分FS-CSA评分的比较中,试验组优于对照组。2组治疗后椎动脉血流参数均有改善,其中试验组治疗后左右椎动脉收缩期峰值血流速度、左右椎动脉平均血流速度较对照组明显提高。在不良反应方面,2组无显著差异,均未出现显著不良反应。结论:采取旋提手法联合舒适牵引的治疗方案,对于椎动脉型颈椎病所引起颈性眩晕的治疗具有良好的临床疗效,安全性及可操作性高,值得进行推广。更多还原

【Abstract】 Objective:To verify the clinical value of rotating and lifting manipulation combined with comfortable traction in the treatment of vertebral artery type cervical spondylosis.Methods:116 patients with cervical vertigo caused by vertebral artery type cervical spondylosis were randomly divided into groups.The experimental group received rotation-lifting technique + comfortable mechanical traction,and the control group used traditional treatment plan of infrared radiation + mechanical traction,once every 2 days,7 times for 1 course of treatment.Analyze and compare the effective rates of the two treatment options,the visual analogue scale for neck pain (VAS) before and after treatment,the evaluation of the vertigo efficacy of the FS-CSA scale,the peak blood flow velocity during vertebral artery systole,the average blood flow velocity and other blood flow parameters,Indicators such as adverse reactions caused by the operation.Results:The effective rate of twisting and lifting technique + comfortable mechanical traction is greater than infrared irradiation + mechanical traction,and the difference is statistically significant.In the clinical efficacy evaluation,the effects of the two groups after treatment were stronger than those before treatment.In the comparison of VAS score and FS-CSA score,the test group is better than the control group.The vertebral artery blood flow parameters in the two groups were improved after treatment.The peak blood flow velocity of the left and right vertebral arteries in the experimental group and the average blood flow velocity of the left and right vertebral arteries were significantly higher than those in the control group after treatment.In terms of adverse reactions,there was no significant difference between the two groups,and no significant adverse reactions occurred.Conclusion:The treatment plan of rotating lifting technique combined with comfortable traction for the treatment of cervical vertigo caused by vertebral artery type cervical spondylosis has a good clinical effect,high safety and operability,and it is worthy of promotion.更多还原
