原发全面强直阵挛癫痫分频段功能MRI度中心度研究
Frequency-Dependent Alterations with Degree Centrality in Idiopathic Generalized Epilepsy with Generalized Tonic-Clonic Seizure Based on Functional MRI
【摘要】 目的探讨原发全面强直阵挛癫痫(IGE-GTCS)不同频段[固有模态函数(IMF1~5)]下静息态功能磁共振成像(f MRI度中心度(DC)的异常。资料与方法前瞻性收集IGE-GTCS患者19例(GTCS组)和22例健康志愿者(对照组),采集静息态fMRI数据。采用互补集合经验模态分解(CEEMD)方法对fMRI振荡信号进行分频计算,比较两组受检者重要节点属性DC的差异。提取组间差异脑区DC与国立医院癫痫发作严重程度量表(NHS3)评分,并进行相关性分析。结果在不同振荡频率下与对照组相比,GTCS组DC值增高的脑区主要集中在默认网络(DMN)的重要节点,DC减低的脑区主要包括双侧丘脑、左侧豆状核及部分DMN脑网络节点(P<0.05)。IMF4亚频段:GTCS组NHS3评分与后扣带回的平均DC呈正相关(r=0.65,P<0.05),与内侧颞叶的平均DC呈负相关(r=-0.51,P<0.05);IMF3亚频段:GTCS组NHS3评分与前扣带回的平均DC呈负相关(r=-0.50,P<0.05);IMF1亚频段:GTCS组NHS3评分与后扣带回呈正相关(r=0.54,P<0.05)。结论 GTCS患者在不同频段下默认网络节点属性DC值异常反映了GTCS患者脑功能整合异常,有助于进一步揭示GTCS的发病机制,并为其后续研究提供参考。
【Abstract】 Purpose To explore the of frequency-dependent [intrinsic mode function(IMF) 1-5] alterations of degree centrality(DC) using resting-state functional magnetic resonance imaging(fMRI) in idiopathic generalized epilepsy with generalized tonic-clonic seizure(IGE-GTCS). Materials and Methods Nineteen patients with IGE-GTCS and 22 healthy volunteers(control group) were prospectively enrolled, and collected the resting-state fMRI data. The fMRI raw data was divided into five characteristic oscillations with distinct frequencies by the data-driven complementary ensemble empirical mode decomposition method, and extracted the DC value of different brain areas, and the national hospital seizure severity scale(NHS3) score was analyzed for correlation. Results Under different oscillation frequencies, compared with the control group, the brain regions with increased DC value in GTCS group were mainly concentrated at important nodes of default model network, and reduced DC mainly include bilateral thalamus, left lentiform nucleus(P<0.05). In GTCS group, NHS3 score was positively correlated with DC value of posterior cingulate gyrus(r=0.65 and 0.54, P<0.05), and negatively correlated with DC value of anterior cingulate gyrus and anterior wedge lobe/middle temporal gyrus(r=-0.51 and r=-0.50, P<0.05). Conclusion The alterations of default network node attribute DC value of GTCS patients under different frequency bands reflects the abnormality of brain function integration of GTCS patients, these alterations are helpful to further reveal the pathogenesis of GTCS and provide a new perspective for its follow-up research.
