胸部CT对新型冠状病毒肺炎的诊断价值

【摘要】 目的分析新型冠状病毒肺炎(COVID-19)的胸部高分辨率CT特征,探讨其对COVID-19的诊断价值。资料与方法纳入102例确诊COVID-19及64例临床诊断COVID-19患者,比较两组患者胸部高分辨率CT征象的差异。结果确诊COVID-19及临床诊断COVID-19组间病灶分布特征、病灶部位、数目、密度及胸腔积液比较,差异均有统计学意义(P<0.05),确诊组多见双肺病灶、周边胸膜下病灶、多发病灶、磨玻璃密度,多无胸腔积液。两组病灶大小、形态、边缘及肿大淋巴结、病灶体积分数比较,差异均无统计学意义(P>0.05)。结论 COVID-19的胸部高分辨率CT有一定的特征,确诊患者较临床诊断患者多见双肺病灶、多发、周边胸膜下、磨玻璃病灶、无胸腔积液等征象,有助于确诊COVID-19。

【Abstract】 Purpose To analyze the high-resolution chest CT features of COVID-19, and to explore the value of high-resolution chest CT in diagnosing COVID-19. Materials and Methods 102 patients with confirmed COVID-19 and 64 patients with clinically diagnosed COVID-19 were enrolled in this study, and the differences in chest high-resolution CT features between the two groups were compared. Results The differences in the lesion distribution characteristics, lesion location, lesion number, lesion density and pleural effusion between COVID-19 confirmed group and clinical diagnosis group were statistically significant(P<0.05). In the COVID-19 confirmed group, there were more lung lesions, peripheral subpleural lesions, multifoci lesions, ground glass density and most showed no pleural effusion. No statistical difference in the lesion size, lesion shape, lesion margin, enlarged lymph nodes and lesion volume fraction was observed between the two groups(P>0.05). Conclusion The high-resolution chest CT manifestations of COVID-19 has certain characteristics, confirmed COVID-19 patients have more signs of double lung lesions, multifoci lesions, peripheral subpleural lesions, ground glass lesions and no pleural effusion compared with clinically diagnosed patients, which is helpful for the diagnosis of COVID-19.
