匹多莫德联合阿昔洛韦对复发性生殖器疱疹患者免疫功能及临床疗效的影响

【摘要】 目的探讨匹多莫德联合阿昔洛韦对复发性生殖器疱疹患者免疫功能及临床疗效的影响。方法选取2017年1月至2018年8月于广州市番禺区慢性病防治站接受治疗的复发性生殖器疱疹患者162例作为研究对象,采用随机数字表法分为对照组与观察组,每组81例。对照组给予阿昔洛韦缓释片口服,观察组在对照组基础上加用匹多莫德分散片,两组患者疗程均为3个月。比较两组患者疗效、免疫功能、炎症介质、复发率及不良反应。结果观察组治疗有效率为96.30%,显著高于对照组的87.65%(P<0.05)。治疗后两组患者免疫球蛋白A(Ig A)、免疫球蛋白G(Ig G)及免疫球蛋白M(Ig M)水平均明显升高,但观察组升高幅度更大(P<0.05)。治疗后两组肿瘤坏死因子-α(TNF-α)、白细胞介素-4(IL-4)及白细胞介素-6(IL-6)水平明显下降,但观察组下降幅度更大(P<0.05)。两组治疗前后比较差异无统计学意义(P>0.05),治疗后两组CD3~+明显上升(P<0.05)。治疗后两组患者CD4~+显著上升,CD8~+显著下降,且观察组CD4~+显著高于对照组,CD8~+显著低于对照组(P<0.05)。观察组随访期间复发率为28.39%,显著低于对照组的64.20%(P<0.05)。治疗期间对照组出现5例(6.17%)不良反应,观察组出现3例(3.70%),两组比较差异无统计学意义(P>0.05)。结论匹多莫德联合阿昔洛韦可显著提高复发性生殖器疱疹疗效,降低复发率,改善患者体液免疫及细胞免疫,缓解炎症反应且不增加不良反应发生率。

【Abstract】 Objective To explore effects of pidotimod combined with acyclovir on immune function and clinical efficacy in patients with recurrent genital herpes. Methods A total of 162 patients with recurrent genital herpes treated in Chronic Disease Prevention and Control Station of Panyu District in Guangzhou from January 2017 to August 2018 were selected for this study. The patients were divided into control group and observation group by the random number table method, with 81 cases in each group. The control group was given acyclovir sustained release tablets orally, and the observation group was given pidotimod dispersible tablets on the basis of the treatment in the control group. The course of treatment in both groups was 3 months. The efficacy, immune function, inflammatory mediators, recurrence rates and adverse reactions were compared between the 2 groups. Results The effective rate in the observation group was 96.30%, which was significantly higher than 87.65% in the control group(P<0.05). After the treatment, the levels of immunoglobulin A(IgA), immunoglobulin G(IgG) and immunoglobulin M(IgM) in the 2 groups were significantly increased, but the increase was greater in the observation group(P<0.05). After the treatment, the levels of tumor necrosis factor-α(TNF-α), interleukin-4(IL-4) and interleukin-6(IL-6) were decreased significantly, but the decrease in the observation group was even greater(P<0.05). The level of CD3~+ in both groups were increased significantly after the treatment(P<0.05), and there was no significant difference between the 2 groups before and after the treatment(P>0.05). After the treatment, the levels of CD4~+ in both groups were increased significantly and the levels of CD8~+ were decreased significantly, and the level of CD4~+ in the observation group was significantly higher than that in the control group, while the level of CD8~+ was significantly lower than that in the control group(P<0.05). During the follow-up period, the recurrence rate in the observation group was 28.39%, which was significantly lower than 64.20% in the control group(P<0.05). During the treatment period, 5 cases(6.17%) of adverse reactions occurred in the control group and 3 cases(3.70%) in the observation group. There was no significant difference between the 2 groups(P>0.05). Conclusion Pidotimod combined with acyclovir can significantly improve the efficacy of recurrent genital herpes, decrease the recurrence rate, improve the patient’s humoral immunity and cellular immunity, as well as relieve the inflammatory response without increasing the incidence of adverse reactions.
