脂肪间充干细胞复合骨软骨一体化支架修复兔骨软骨缺损的实验研究

【摘要】 目的探讨脂肪间充质干细胞复合骨软骨一体化支架修复兔骨软骨缺损的效果及其临床应用的可能性和优势。方法成年新西兰大白兔构建骨软骨缺损动物模型。A组为空白对照组,仅做缺损手术;B组为骨软骨一体化支架组,植入不含细胞的支架;C组为负载脂肪间充质干细胞的骨软骨一体化支架组。术后6、12周取材,进行大体观察,根据国际软骨修复协会(ICRS)组织学评分标准评分,HE染色和番红O-固绿染色进行组织学评估。结果术后6周,一体化支架组与ADSCs复合一体化支架组的ICRS评分显著高于空白对照组(P<0.05);术后12周,一体化支架组与ADSCs复合支架组的ICRS评分仍显著高于空白对照组(P <0.05),且ADSCs复合一体化支架组的评分显著高于一体化支架组(P <0.05)。HE染色、甲苯胺蓝与番红O-固绿染色表明,一体化支架组和ADSCs复合一体化支架组的修复明显优于空白对照组,且ADSCs复合一体化支架组的修复较一体化支架组更优。结论骨软骨一体化支架能够促进兔膝关节骨软骨缺损的修复,但修复效果有限。ADSCs能够明显提高支架的作用,ADSCs复合一体化支架是此次动物实验中最佳的骨软骨修复方式。

【Abstract】 Objective To investigate the effect of adipose mesenchymal stem cells(ADSCs) combined with osteochondral integrated scaffold on the repair of osteochondral defects in rabbit knee. Methods New Zealand adult rabbits were divided into three groups as follows. Group A was the control group consisting of rabbits with cartilage defects in which no repair procedures were performed. Group B was treated with the cell-free osteochondral integrated scaffold. Group C was treated with the scaffold with ADSCs. The animals were killed at 6 and 12 weeks after operation and histological assessments were performed. The 6 and 12-week specimens were evaluated using the International Cartilage Repair Society(ICRS) histological scoring system. Results The ICRS scores of both the osteochondral integrated scaffold only group and the ADSCs-scaffold group were significantly higher than that of the control group at 6 and 12 weeks after surgery(P < 0.05). Additionally, the score of the ADSCs-scaffold group was significantly higher than that of the osteochondral integrated scaffold only group at 12 weeks(P < 0.05). Furthermore, the histological assessments with hematoxylin-eosin staining, toluidine blue dyeing and safranin O-fast green staining revealed significantly improved osteochondral regeneration in the ADSCs-scaffold group when compared with the scaffold only group and the control group. Conclusion The scaffold does repair the osteochondral defect, but the protective effect is limited. After ADSCs is added, the repairing capacity of osteochondral integrated scaffold is significantly enhanced. ADSCs-scaffold may be a better choice for osteochondral regeneration in this animal experiment.
