地黄饮子联合脑-机接口训练系统对脑梗死恢复期上肢功能障碍的影响

【摘要】 目的:观察地黄饮子联合脑-机接口(Brain-Computer Interface,BCI)训练系统对脑梗死恢复期上肢功能障碍的影响,探寻脑梗死恢复期上肢功能障碍的有效治疗措施。方法:选取86例脑梗死恢复期患者随机分为两组,联合组和对照组各43例。对照组口服地黄饮子,联合组采用地黄饮子联合BCI训练系统治疗。治疗前和3个疗程后用简化的上肢评价量表(FuglMeyer Assessment,FMA)、改良Barthel指数(Modified Barthel Index,MBI)、徒手肌力检查法(Manual Muscle Testing,MMT)对两组治疗结果进行评分和临床疗效评定。并采用经颅多普勒(Transcranial Doppler,TCD)监测患者治疗前后收缩期血流峰值速度(Peak Systolic Velocity,Vs)、舒张期末血流速度(End Diastolic Velocity,Vd)、阻力指数(Resistance Index,RI)、血管搏动指数(Pulsatility Index,PI)的变化情况。结果:FMA、MBI、MMT、TCD评分两组治疗后均优于治疗前,联合组优于对照组,两组比较差异有统计学意义(均P <0.05);总有效率联合组93.02%(31/43),对照组72.09%(40/43),两者差异有统计学意义(P <0.05)。结论:地黄饮子联合BCI训练系统能有效地治疗脑梗死恢复期上肢功能障碍,显著改善患者的生活质量,联合组明显优于对照组。

【Abstract】 Objective: To observe the effect of Rehmannia decoction (地黄饮子) combined with BCI training system on upper limb dysfunction in the recovery period of cerebral infarction, and to explore the effective treatment measures of upper limb dysfunction in the recovery period of cerebral infarction. Method: 86 patients with cerebral infarction in recovery period were randomly divided into two groups, the combined group and the control group, with 43 cases in each group. The control group was treated with Rehmannia decoction and the combined group was treated with Rehmannia decoction combined with BCI training system. Before and after three courses of treatment, the two groups were assessed with the simplified upper extremity assessment FMA, modified Barthel Index MBI and manual muscle testing MMT. Transcranial Doppler TCD was used to monitor the changes of VS, VD, RI and PI before and after treatment. Results: The scores of FMA, MBI, MMT and TCD in the combined group were higher than those in the control group after treatment(P<0.05), and the total effective rate in the combined group was 93.02%(31/43), and 72.09%(40/43) in the control group(P<0.05). Conclusions: Rehmannia decoction combined with BCI training system can effectively treat the upper limb dysfunction in the recovery period of cerebral infarction, and significantly improve the quality of life of patients, the combined group is significantly better than the control group.
