【摘要】 目的:研究腔镜Ivor-Lewis治疗胸中下段食管癌对消化功能及免疫功能的影响。方法:选取2016年1月-2019年8月本院收治的中下段食管癌患者46例,按随机数字表法将其分为对照组和研究组,每组23例。对照组予以McKeown术式治疗,研究组予以腔镜Ivor-Lewis治疗。比较两组临床情况及两组手术前后消化功能情况、免疫功能。结果:研究组术后下床活动时间、术后住院时间均短于对照组,术后24 h疼痛评分低于对照组,差异均有统计学意义(P<0.05)。术后3 d,研究组的胃液量、胃排空率及基础胃酸分泌量均高于对照组,差异均有统计学意义(P<0.05)。术后1周,两组IgA及IgG水平均低于治疗前,但研究组IgA及IgG水平均高于对照组,差异均有统计学意义(P<0.05)。结论:腔镜Ivor-Lewis治疗胸中下段食管癌对食管消化功能及免疫功能的影响更小,术后疼痛更轻,值得推广。更多还原

【Abstract】 Objective:To study the effect of endoscopic Ivor-Lewis on digestive function and immune function in the treatment of middle and lower thoracic esophageal cancer.Method:A total of 46 patients with middle and lower esophageal cancer admitted to our hospital from January 2016 to August 2019 were selected,they were divided into control group and study group according to random number table,23 cases in each group.The control group was treated with McKeown,the study group was treated with Ivor-Lewis.The clinical situation,digestive function and immune function of the two groups before and after the operation were compared.Result:The time of getting out of bed and hospitalization in the study group were shorter than those in the control group,and the pain score of 24 hours after operation was lower than that in the control group,the differences were statistically significant (P<0.05).3 days after operation,the gastric juice volume,gastric emptying rate and basic gastric acid secretion of the study group were higher than those of the control group,the differences were statistically significant (P<0.05).1 week after operation,the levels of IgA and IgG in both groups were lower than those before treatment,but the levels of IgA and IgG in the study group were higher than those in the control group,the differences were statistically significant (P<0.05).Conclusion:Endoscopic Ivor-Lewis treatment of middle and lower thoracic esophageal cancer has less effect on the digestive function and immune function of esophagus,and less postoperative pain,which is worth popularizing.更多还原
