术前呼吸功能锻炼对腹部手术患者术后肺部并发症影响的Meta分析

【摘要】 目的评价术前呼吸功能锻炼对腹部手术患者术后肺部并发症的影响。方法使用Cochrane协作网提供的系统评价方法,通过计算机检索中文数据库(万方、中国知网、维普)和外文数据库(PubMed、Cochrane Library、Embase、Sciencedirect)建库至2019年8月期间发表的有关腹部手术患者术前采取呼吸功能锻炼指导的随机对照实验。筛选文献、提取资料并进行文献质量评价,采用RevMan 5.3软件进行Meta分析。结果最终纳入9篇文献,共1502例患者。Meta分析结果显示,术前呼吸功能锻炼指导能够显著降低腹部手术患者术后肺部并发症的发生率(OR=0.26, 95%CI:0.19~0.36,P<0.051),降低肺部感染的发生率(OR=0.29, 95%CI:0.16~0.50,P<0.01),降低肺不张的发生率(OR=0.22, 95%CI:0.05~0.89,P<0.05)。结论术前呼吸功能锻炼指导可以有效降低腹部手术患者肺部并发症、肺部感染及肺不张的发生风险。

【Abstract】 Objective To evaluate the effect of preoperative respiratory function exercise on postoperative pulmonary complications in patients with abdominal surgery. Methods Databases including CNKI, Wanfang Data, VIP, PubMed, Cochrane Library, Embase and Sciencedirect were electronically searched. Randomized controlled trials describing the effect of preoperative respiratory function exercise on postoperative pulmonary complications in patients with abdominal surgery published between the establishment of database and August 2019. Two researchers independently screened literature, extracted data, and strictly assessed the methodological quality of the included studies. Data was analyzed by RevMan 5.3 software. Results A total of 9 articles with 1502 patients were included in the review. The meta-analysis showed that the incidence of postoperative pulmonary complications in patients with abdominal operation was significantly reduced(OR=0.26, 95%CI: 0.19-0.36, P<0.01), and the incidence of pulmonary infection was reduced(OR=0.29, 95% CI: 0.16-0.50, P<0.01), and the incidence of atelectasis was reduced(OR=0.22, 95%CI: 0.05-0.89, P<0.05). Conclusion Preoperative respiratory function exercise can reduce the risk of postoperative pulmonary complications in patients with abdominal operation.
