【摘要】 目的评价高渗盐水与甘露醇治疗脑损伤性颅高压的安全性。方法系统检索知网、万方、生物医学、维普数据库,筛选高渗盐水与甘露醇治疗脑损伤性颅高压的随机对照实验,并提取相关数据。采用RevMan 5.3软件对相关数据进行合并分析。结果共纳入研究20项,合计1439名病人。Meta分析结果显示,与对照组(甘露醇)比较,实验组(高渗盐水)在血肌酐水平、总尿量、不良事件、血钠浓度的差异具有统计学意义,尿素氮水平组间差异无统计学意义。结论高渗盐水用于管理非神经危重者颅内压相对安全。

【Abstract】 Objective To evaluate the safety of hypertonic saline and mannitol in the treatment of intracranial hypertension after brain injury. Methods The Chinese databases(CNKI, Wanfang, Biomedicine, and Weipu) were systematically searched. The randomized controlled trials of hypertonic saline and mannitol in the treatment of intracranial hypertension caused by brain injury was extracted for relevant data. The pooled analysis of the relevant data was conducted by RevMan 5.3 software. Results A total of 20 studies were included, with a total of 1439 patients. The Meta-analysis results showed that compared with the control group(mannitol), the experimental group(hypertonic saline) had statistically significant differences in serum creatinine level, total urine output, adverse events, and serum sodium concentrations, and no significant difference in urea nitrogen levels. Conclusion Hypertonic saline is relatively safe to manage intracranial pressure in non-neurologically critical patients.
