妊娠及产褥期新型冠状病毒肺炎合并静脉血栓栓塞症患者的合理用药及药学服务

【摘要】 新型冠状病毒肺炎(COVID-19)患者尤其是重症类型,疾病后期机体凝血/纤溶系统功能紊乱十分常见,极易诱发或加重深静脉血栓形成(DVT),甚至发生肺栓塞(PE)等严重致死性临床结局。COVID-19合并妊娠及产褥期这一类高危人群,其机体一系列生理、血流动力学改变会增加患者发生静脉血栓栓塞症(VTE)的风险;同时VTE患者血液高凝和纤溶酶原亢进状态容易造成机体免疫识别障碍,有助于新型冠状病毒(SARS-CoV-2)的免疫逃逸,使COVID-19患者病程延长,两种疾病相互影响。因此采取及时有效的预防治疗方案能减少妊娠期COVID-19患者发生VTE的风险、缩短病程、改善预后。本文结合VTE合理用药相关研究,探讨妊娠及产褥期COVID-19合并VTE患者的抗病毒、抗凝及溶栓治疗方案与药学监护服务,以期为这类特殊患者合理用药提供参考。

【Abstract】 The severe and critical corona virus disease 2019 (COVID-19) tends to disorder the coagulation/fibrinolysis system,which might easily induce or aggravate deep venous thrombosis (DVT),sometimes even causing to pulmonary embolism (PE).Due to physiological and hemodynamic changes,pregnant and postpartum women with COVID-19 are at increased risk of venous thromboembolism (VTE).Conversely,patients with VTE have the hypercoagulable state of blood,which can prolong the course of COVID-19.So a timely and effective treatment plan should be made to shorten the course of disease and improve prognosis.Combined with the research on the rational drug use of VTE,we discussed the antiviral,anticoagulant and thrombolytic therapy and pharmaceutical care for pregnant and postpartum women with COVID-19 and VTE,to provide reference for reasonable medication regimen for these patients.
