莱菔硫烷预先给药对兔单肺通气时肺损伤Nrf2蛋白变化及抗肺损伤的机制

【摘要】 目的探讨莱菔硫烷(SFN)预先给药对兔单肺通气(OLV)时肺损伤核因子(NF)-E2相关因子(Nrf)2蛋白变化及抗肺损伤的机制。方法取24只健康雄性大白兔,按照随机数字表法分为双肺通气(TLV)组、OLV组及SFN预先给药OLV(SFN)组各8只,SFN组于OLV前30 min以5 mg/kg SFN行腹腔注射,OLV组和TLV组同时点给予等量羟甲基纤维素钠的溶媒。比较3组通气开始(T0)、通气1 h(T1)、2 h(T2)、3 h(T3)的氧合指数及光镜下各组病理学组织结果、相关指标水平。结果 T2时OLV、SFN组氧合指数明显低于TLV组,T3时OLV、SFN组明显低于TLV组,但SFN组明显高于OLV组,T2、T3时OLV、SFN组明显低于T0时(均P<0.05);OLV组、SFN组Nrf2、血红素氧合酶(HO)-1蛋白表达、肺损伤评分、湿/干重比值(W/D)明显高于TLV组,SFN组Nrf2、HO-1蛋白表达明显高于OLV组,肺损伤评分、W/D低于OLV组(均P<0.05);OLV组、SFN组丙二醛(MDA)表达及肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6、IL-8浓度明显高于TLV组,超氧化物歧化酶(SOD)活性明显低于TLV组,SFN组MDA表达及TNF-α、IL-6、IL-8浓度明显低于OLV组,SOD活性明显高于OLV组(均P<0.05)。结论 SFN预先给药通过促进肺组织Nrf2蛋白表达,抑制脂质过氧化反应及炎性反应来减轻OLV所诱发的肺损伤。

【Abstract】 Objective To investigate the effect of sulforaphane on the changes of Nrf2 protein in lung injury induced by one-lung ventilation in rabbits and the mechanism of anti-lung injury induced by sulforaphane. Methods 24 healthy male rabbits were randomly divided into three groups: TLV group(double lung ventilation group), OLV group(right lung one lung ventilation group) and SFN group(sulforaphane pre-administered single lung ventilation group), with 8 rabbits in each group. SFN group was intraperitoneally injected with 5 mg/kg SFN 30 min before one-lung ventilation, and the same amount of sodium hydroxymethyl cellulose solution was administered simultaneously in the OLV group and TLV group. The oxygenation index of ventilation beginning(T0), ventilation 1 hour(T1), ventilation 2 hours(T2) and ventilation 3 hours(T3) in three groups were compared. Results The oxygenation index of OLV and SFN groups at T2 was lower than that of TLV group(P<0.05), and that of OLV and SFN groups at T3 was lower than that of TLV group, but that of SFN group was higher than that of OLV group(P<0.05). The oxygenation index of OLV and SFN groups at T2 and T3 was significantly lower than that at T0(P<0.05). The expression of Nrf2, HO-1 protein and the ratio of W/D in OLV group and SFN group were higher than those in TLV group(P<0.05). The expression of Nrf2 and HO-1 protein in SFN group were higher than those of OLV group, and the lung injury score and W/D ratio were lower than those of OLV group(P<0.05). The expression of MDA, the concentration of TNF-α, IL-6 and IL-8 in OLV and SFN groups were higher than those of TLV group, while the SOD activity in TLV group was lower than that of TLV group(P<0.05). The MDA expression and TNF-α, IL-6 and IL-8 concentrations in TLV group were significantly lower than those of OLV group(P<0.05). The activity of SOD was higher in OLV group than that of OLV group(P<0.05). Conclusions Sulforaphane pretreated with sulforaphane could reduce the lung injury induced by OLV by promoting the expression of Nrf2 protein and inhibiting lipid peroxidation and inflammatory reaction.
