15例新型冠状病毒肺炎死亡患者临床实验室检查结果的特点分析

【摘要】 目的对重症新型冠状病毒肺炎(简称新冠肺炎)死亡患者临床实验室检查结果的特点进行分析。方法收集因新型冠状病毒肺炎死亡患者的一般情况,和患者初入ICU时(T1)及死亡前最近一次(T2)的相关临床实验室检测数据,分析其特征特点。结果重症新冠肺炎患者死亡前氧合状态没有明显改善,与T1比较,T2二氧化碳分压(PCO2)、乳酸脱氢酶(LDH)、心肌肌钙蛋白I(cTnI)、脑钠肽(BNP)、尿素氮(BUN)、谷丙转氨酶(ALT)明显升高(P <0.05),氧分压(PO2)、C反应蛋白(CRP)、淋巴细胞计数(LY)、肌酐(Cr)、白蛋白(ALB)、谷草转氨酶(AST)变化不明显。结论缺氧状态下新冠病毒对心脏损害可能是导致新冠肺炎患者死亡的主要原因;cTnI、BNP、LDH或可参考作为预测患者预后的指标,提请临床关注。

【Abstract】 Objective To analyze the characteristics of clinical laboratory examination results of the patients who died of severe new coronavirus pneumonia.Methods The general situation of the patients who died of the new coronavirus pneumonia,and the relevant clinical laboratory test data of the patients when they were transferred to ICU(T1) and before death(T2) were collected and analyzed.Results There was no significant improvement in oxygenation before death in the patients with severe new coronavirus pneumonia,and PCO2,lactate dehydrogenase(LDH),cardiac troponin I(cTnI),brain natriuretic peptide(BNP),urea nitrogen(BUN) and alanine transaminase(ALT) were significantly increased compared with those of T1(P <0.05).PO2,C-reactive protein(CRP),lymphocyte(LY),creatinine(Cr),albumin(ALB) and aspartate aminotransferase(AST) did not change obviously.Conclusion The heart damage caused by novel coronavirus under hypoxia may be the main cause of death in patients with novel coronavirus pneumonia.cTnI,BNP and LDH can be referred as indicators of the patients’ prognosis to draw clinical attention.
