人工真皮联合自体皮片修复创伤后骨和/或肌腱外露创面的临床研究

【摘要】 目的探讨人工真皮联合自体皮片修复创伤后骨和/或肌腱外露创面的临床效果。方法选取2017年1月至2019年12月北京积水潭医院烧伤科收治的创伤后骨和/或肌腱外露的24例患者为研究对象,采用人工真皮联合自体皮片进行修复。治疗方法为Ⅰ期创面扩创,彻底清除坏死组织后,创面表面移植人工真皮,待2~3周人工真皮血管化良好、外露的骨和/或肌腱完全被类真皮组织覆盖后,Ⅱ期行薄层自体皮移植手术修复创面。结果 24例患者骨和/或肌腱外露创面移植的人工真皮血管化良好,移植的自体皮成活良好,创面得到有效修复,供皮区瘢痕较轻。结论人工真皮和自体皮移植修复创伤后骨和/或肌腱外露创面方法简单,修复效果好,供皮区损伤轻,值得临床应用。

【Abstract】 Objective To investigate the clinical effect of repairing exposed bone and/or tendon wounds after trauma with artificial dermis and autologous skin graft. Method 24 patients with exposed bone and/or tendon exposed from the Department of Burns, Beijing Jishuitan Hospital from January 2017 to December 2019 were selected as the research subjects and repaired with artificial dermis and autologous skin graft. The treatment method was to debride the wound in stage Ⅰ. After the necrotic tissue was completely removed, the artificial dermis was transplanted on the surface of the wound. The artificial dermis was well vascularized in 2-3 weeks, and the exposed bone and/or tendon was completely covered with dermal-like tissue. In the end, we made skin grafts to repair the wound. Result In 24 patients, the grafts with exposed bone and/or tendon exposed wounds had good vascularization of the artificial dermis, and the transplanted autogenous skin survived well, the wounds were effectively repaired, and the scars in the donor site were lighter. Conclusion The artificial dermis and autologous skin grafts can be used to repair exposed bone and/or tendon exposed wounds after trauma. The method is simple, the repair effect is good, and the donor site is lightly damaged.
