Smartpatch长时程动态心电图对心房颤动消融术后复发的监测及相关因素分析

【摘要】 目的使用贴附式可充电长时程动态心电图记录仪Smartpacth,进行心房颤动(AF)消融术后心电监测,定期长时程随访,探究该类患者AF术后复发情况及复发因素的分析。方法选取2018年7月至2018年12月到普陀区中心医院首次行AF消融手术的52例患者,在术后第1、6、12、26周分别按期佩戴使用Smartpacth进行7天的心律监测。结果 52例患者中共检测出29例(55.8%)早期复发。术后第26周复发4例(7.7%)。在多因素Logistic回归分析中,高血压是术后第26周复发的预测因子(P=0.0344,OR 13.00)。McNemar检验发现术后第12周与第26周的一致性高(KAPPA 0.8785,P=0.3173)。结论高血压是术后第26周复发的预测因子;术后第12、26周的复发率一致性高。

【Abstract】 Objective Smartpacth was used to monitor the ECG after ablation of atrial fibrillation(AF), and the regular long-term follow-up was carried out to explore the occurrence of arrhythmia and the recurrence of AF. Methods From July 2018 to December 2018, 52 patients who went to Putuo District Central Hospital for the first time to undergo AF ablation were selected to wear smartpacth on schedule for 7-day rhythm monitoring at the 1 st, 6 th, 12 th and 26 th weeks after operation. Results In 52 patients, 29(55.8%) had early recurrence. Recurrence occurred in 4 cases(7.7%) at the 26 th week. In multivariate Logistic regression analysis, hypertension was the predictor of postoperative recurrence at the 26 th week(P= 0.0344, or 13.00). McNemar test showed a high consistency between the 12 th and 26 th week(kappa 0.8785, P=0.3173). Conclusion Hypertension is the predictor of postoperative recurrence at the 26 th week, and the recurrence rate is consistent at the 12 th and 26 th week.[Chinese Journal of Cardiac Pacing and Electrophysiology,2020,34(2):124-127]
