降尿酸治疗对老年2~5期慢性肾脏病合并高尿酸血症患者肾功能的影响
Effect of uric acid therapy on renal function in elderly patients with chronic kidney disease complicated with hyperuricemia
【摘要】 目的探讨降尿酸治疗对老年2~5期慢性肾脏病(CKD)合并高尿酸血症患者肾功能的影响。方法根据患者肾小球滤过率(GFR)将2019年1月~2020年1月本院治疗的110例2~5期CKD合并高尿酸血症患者,按GFR值分成A组[GFR值30~89 ml/min(60~89 ml/min为2期,30~59 ml/min为3期)74例]与B组[GFR值0~29 ml/min(15~29 ml/min为4期,<15 ml/min为5期)36例]。两组采用非布司他治疗。比较两组的血尿酸(SUA)达标率,各组治疗前后肾动能[血肌酐(SCr)、SUA、尿素氮(BUN)]变化及所有患者用药的不良反应。结果治疗6个月后,两组降尿酸达标率均在90%以上(P>0.05);治疗后,A、B两组各时点SCr、SUA、BUN水平降低,其中A组改善效果更好(P<0.05)。结论非布司他对2~3期CKD合并高尿酸血症的治疗效果良好,可有效改善肾功能,延缓病情进展,且安全性较高,值得推广。
【Abstract】 Objective To investigate the effect of uric acid lowering therapy on renal function in elderly patients with2-5 stage chronic kidney disease with hyperuricemia. Methods According to the patient′ s glomerular filtration rate(GFR), 110 patients with stage 2 to 5 chronic kidney disease and hyperuricemia who were treated in our hospital from January 2019 to January 2020 were selected and divided into group A(74 cases), GFR value was between 30-89 ml/min(60-89 ml/min was stage 2, 30-59 ml/min was stage 3) and group B(36 cases), GFR value was between 0-29 ml/min(15-29 ml/min was 4 stages, <15 ml/min was 5 stages, according to GFR value. Both groups were treated with Febuxostat. The blood uric acid(SUA) attainment rate was compared between the two groups. the changes in renal kinetic energy(blood creatinine [SCr], SUA, urea nitrogen [BUN]) before and after treatment and adverse drug reactions in all patients were compared between the two groups Results After 6 months of treatment, the uric acid lowering compliance rate of both groups was above 90%(P>0.05); after treatment, the levels of SCr, SUA, and BUN at each time point in groups A and B decreased, and the improvement effect of group A was better(P<0.05). Conclusion Febuxostat has a good effect on the treatment of stage 2-3 chronic kidney disease with hyperuricemia, can effectively improve renal function, delay the progression of the disease, and has high safety and is worth promoting.
