低温等离子射频选择性消融联合下鼻甲骨折外移治疗慢性鼻炎的临床效果

【摘要】 目的探讨低温等离子射频选择性消融联合下鼻甲骨折外移治疗慢性鼻炎的临床效果。方法选取2018年4月~2019年3月本院收治的80例慢性鼻炎患者作为研究对象,按照随机数字表法分为观察组(40例)及对照组(40例)。观察组采用低温等离子射频选择性消融联合下鼻甲骨折外移术,对照组采用低温等离子射频消融联合下鼻甲成形术。比较两组患者的手术时间、术中出血量、术前及术后6个月主观症状视觉模拟量表(VAS)评分、术前及术后6个月双侧下鼻甲与鼻中隔距离、并发症(空鼻综合征、鼻腔粘连)发生情况。结果观察组的手术时间短于对照组,差异有统计学意义(P<0.05);观察组的术中出血量少于对照组,差异有统计学意义(P<0.05);两组的术后鼻塞、头昏或头痛、影响生活工作或学习VAS评分低于本组术前,差异均有统计学意义(P<0.05);观察组的术后鼻塞、头昏或头痛、影响生活工作或学习VAS评分低于对照组,差异均有统计学意义(P<0.05);两组的术前双侧下鼻甲与鼻中隔距离比较,差异无统计学意义(P>0.05);两组术后的双侧下鼻甲与鼻中隔距离大于术前,差异有统计学意义(P<0.05);观察组术后的双侧下鼻甲与鼻中隔距离大于对照组,差异有统计学意义(P<0.05)。两组均无空鼻综合征、鼻腔粘连等并发症发生。结论低温等离子射频选择性消融联合下鼻甲骨折外移治疗慢性鼻炎操作简单,手术时间短,术中出血量少,疗效肯定,并发症少,是表现为下鼻甲骨增生及下鼻甲黏膜肥厚的慢性鼻炎患者的较理想治疗方法。

【Abstract】 Objective To investigate the clinical effect of low temperature plasma radiofrequency selective ablation combined with inferior turbinate fracture in the treatment of chronic rhinitis. Methods A total of 80 patients with chronic rhinitis admitted to our hospital from April 2018 to March 2019 were selected as research objects. According to the method of random number table, they were divided into the observation group(40 cases) and the control group(40 cases). The patients in the observation group were treated with low temperature plasma radiofrequency selective ablation combined with inferior turbinate fracture extraversion, while those in the control group were treated with low temperature plasma radiofrequency ablation combined with inferior turbinate plasty. The operative time, intraoperative hemorrhage, preoperative and postoperative 6-month subjective symptom visual analogue scale(VAS), distance between bilateral inferior turbinate and nasal septum, complications(empty nose syndrome, nasal cavity adhesion) were compared between the two groups. Results The operation time of the observation group was shorter than that of the control group, the difference was statistically significant(P<0.05). The amount of bleeding in the observation group was less than that in the control group, the difference was statistically significant(P<0.05). The VAS scores of nasal obstruction,dizziness or headache, influence on life, work or learning vas of the two groups after operation were lower than those before operation, the difference were statistically significant(P<0.05). The VAS score of nasal obstruction, dizziness or headache, influence on life, work or learning vas in the observation group after operation were lower than those in the control group, the difference were statistically significant(P <0.05). There was no significant difference in the distance between bilateral inferior turbinate and nasal septum between the two groups(P>0.05). The distance between bilateral inferior turbinate and nasal septum in the two groups after operation was greater than that before operation, the difference was statistically significant(P<0.05). The distance between bilateral inferior turbinate and nasal septum in the observation group after operation was greater than that in the control group, the difference was statistically significant(P<0.05). There were no empty nose syndrome, nasal adhesion and other complications in the both groups. Conclusion Low temperature plasma radio frequency selective ablation combined with inferior turbinate fracture transfer is a reasonable treatment for chronic rhinitis patients with simple operation, which can shorten operation time, with less bleeding during operation, definite curative effect and fewer complications. It is an ideal treatment for chronic rhinitis patients with inferior turbinate bone hyperplasia and inferior turbinate mucosa hypertrophy.
