二维和经食管三维超声对二尖瓣脱垂精细定位的比较研究
The Comparative Study of Two-dimensional and Three-dimensional Transesophageal Echocardiography in Patients With Fine Positioning of Mitral Valve Prolapse
【摘要】 目的探讨二维经胸超声心动图(TTE)、实时三维经胸超声心动图(RT-3D-TTE)及实时三维经食管超声心动图(RT-3D-TEE)评价二尖瓣脱垂精细定位的临床价值。方法回顾性分析68例二尖瓣脱垂患者,采取TTE、RT-3D-TTE及RT-3D-TEE观察二尖瓣瓣叶和腱索的形态、回声、启闭及血流动力学情况。并根据手术结果,对各种方法精细定位二尖瓣脱垂进行比较研究。结果 TTE、RT-3D-TTE及RT-3D-TEE对二尖瓣脱垂精细定位准确率分别为50%、91.7%、100%。TTE对单瓣叶单区域定位率为100%,多区域为33.3%;RT-3D-TTE对单瓣叶单区域定位率为100%,多区域为88.9%;RT-3D-TEE无论单瓣叶单区域或双瓣叶多区域精细定位率均为100%。结论 TTE对单瓣叶单区域脱垂定位较为准确;但前、后叶联合多区域脱垂时,RT-3D-TTE及RT-3D-TEE精细定位分区更具优势。
【Abstract】 Objective To analyze the ultrasonic characteristics of mitral valve prolapse(MVP) and to explore the value of transthoracic echocardiography(TTE),real-time three-dimensional transthoracic echocardiography(RT-3 D-TTE) and real-time three-dimensional transesophageal echocardiography(RT-3 D-TEE) in the fine positioning diagnosis of MVP. Methods 68 patients with MVP diagnosed in our hospital were analyzed retrospectively. The morphology, motion and hemodynamics of mitral valve apparatus were observed by 2 D and RT-3 D TTE and RT-3 D-TEE. All the results were compared with surgical and pathological findings. Results The accuracy of TTE, RT-3 D-TTE, and RT-3 D-TEE for fine positioning of mitral valve prolapse were 50%, 91.7%, and 100%, respectively. The fine localization rate of TTE for single-leaf single-region prolapse was 100%, and the fine localization rate of multi-region prolapse was 33.3%; The fine localization rate of multi-region prolapse is 88.9% and single-region prolapse was 100%. The fine localization rate of prolapse in three-dimensional TEE was 100% regardless of single-leaf single-region or multi-region prolapse. Conclusions RT-3 D TTE and RT-3 D TEE are more accurate than conventional TTE in the fine localization diagnosis of MVP when multi-region prolapse.
