【摘要】 目的探讨家庭医生团队式管理对社区糖尿病患者进行健康教育干预的效果。方法选择2018年在基层医疗卫生服务机构就诊的677例糖尿病患者进行糖尿病相关知识问卷调查,对实验组和对照组分别进行健康教育干预,给予糖尿病患者传统方法治疗,同时检测两组患者血糖值并进行统计分析。结果实验组的糖尿病相关知识知晓率显著高于对照组(P<0.05),实验组糖尿病患者血糖水平(6.51 mmol/L)经过家庭医生团队式管理后明显低于对照组(7.66 mmol/L),干预前后比较差异有统计学意义(t=10.728,P<0.05)。不同年龄、不同性别糖尿病患者家庭医生团队式管理的效果差异显著(P<0.05)。结论为提高糖尿病患者自我保健和自我管理效果,应加强健康教育投入,以利于提高社区糖尿病的预防与控制水平。家庭医生团队式管理模式在社区糖尿病患者中应用具有良好的效果,值得大力应用与推广。更多还原

【Abstract】 OBJECTIVE To probe into the effect of health education intervention of family doctor team management on community diabetes patients. METHODS 677 patients with diabetes who were treated in primary medical and health service institutions in 2018 were selected to carry out a questionnaire survey of diabetes-related knowledge. The experimental group and the control group were given health education intervention and the patients with diabetes were treated by traditional methods at the same time, the blood glucose of the two groups were measured and analyzed statistically. RESULTS The awareness rate of diabetes related knowledge in the experimental group was significantly higher than that in the control group(P<0.05). The Blood Glucose index of the experimental group was 6.51 mmol/L,which was significantly lower than that of the control group(7.66 mmol/L)after the family doctor team management There was significant difference before and after intervention t 10.728,(P<0.05). The effect of team management by family doctors in different age and gender diabetes patients was significant(P<0.05). CONCLUSION In order to improve the effect of self-care and self-management of diabetic patients,health education should be strengthened to improve the prevention and control of diabetes in community. Family Doctor Team Management Model in the community of diabetes patients presented good results, which was worth to vigorously apply and promote.更多还原
