【摘要】 目的探讨超声胃镜(EUS)联合磁共振(MR)对胰腺导管内乳头状黏液瘤(IPMN)的诊断价值。方法选取2015年7月~2019年6月我院收治的疑似胰腺占位性病变患者168例,行MR和EUS检查。根据最终诊断结果,比较两种检查方式单独及联合诊断IPMN的灵敏度、特异度和准确度。结果 168例患者中最终诊断IPMN 18例(10.71%),其中以患者病理结果作为最终诊断结果确诊15例,随访6个月作为最终诊断结果确诊3例;其他150例(89.29%)。根据患者最终诊断结果,EUS灵敏度、特异度和准确度分别为83.33%、92.67%、91.67%,MR分别为50.00%、85.33%、81.55%,EUS+MR分别为100.00%、98.67%、98.81%,EUS和MR联合诊断IPMN的灵敏度、特异度和准确度明显高于其单独诊断(P<0.05)。结论 EUS联合MR应用于IPMN诊断中,能明显提高灵敏度、特异度和准确度,具有较高的临床应用价值。更多还原

【Abstract】 Objective To explore the diagnostic value of EUS combined with MR for IPMN. Methods A total of 168 patients with suspected pancreatic space-occupying lesions who were admitted to our hospital from July 2015 to June2019 were selected. They were given MR and EUS for examinations. According to the final diagnosis results, the sensitivity, specificity and accuracy of the two examinations performed alone and in combination in the diagnosis of IPMN were compared. Results 18 cases(10.71%) of 168 patients were finally diagnosed as IPMN. Among them, 15 cases were diagnosed based on the pathological results of the patients as the final diagnosis, 3 cases were diagnosed based on the6-month follow-up visits as the final diagnosis; the other 150 cases(89.29%) were diagnosed otherwise. According to the patients’ final diagnosis, the sensitivity, specificity, and accuracy of EUS were 83.33%, 92.67%, and 91.67% respectively, those of MR were 50.00%, 85.33%, and 81.55% respectively, and those of EUS +MR were 100.00%,98.67%, and 98.81% respectively. The sensitivity, specificity and accuracy of the combined EUS and MR in the diagnosis of IPMN were significantly higher than those of respective diagnosis alone(P<0.05). Conclusion The application of EUS combined with MR in the diagnosis of IPMN can significantly improve the sensitivity, specificity and accuracy,and has a high clinical application value.更多还原
