早期营养支持策略对早产儿神经行为发育与体格发育的影响

【摘要】 目的探讨早期营养支持策略对早产儿神经行为发育与体格发育的影响。方法选取2016年6月~2018年6月期间住院治疗的早产儿86例,随机分为观察组与对照组,各43例。对照组早产儿均根据体重规划每天所需能量予以常规的营养支持,观察组早产儿在此治疗的基础上采用早期营养支持策略干预。两组疗程均到纠正胎龄40周。观察两组早产儿体重增长情况、恢复出生体重时间及并发症发生情况,并在纠正胎龄40周时比较两组神经行为发育检测(NBNA)的差异。结果观察组早产儿恢复出生体质量时间、营养支持时间和住院时间均明显少于对照组(P<0.05),观察组早产儿恢复出生体质量后体重增长量明显大于对照组(P<0.05);观察组早产儿NBNA测试各项评分及总分均明显高于对照组(P<0.05);观察组早产儿并发症发生率明显低于对照组(χ~2=4.02,P<0.05)。结论早期营养支持策略能增快早产儿神经行为发育与体格发育速度,缩短营养支持时间与住院时间,减少并发症发生。

【Abstract】 Objective To investigate the effects of early nutritional support strategies on neurobehavioral development and physical development in premature infants. Methods A total of 86 preterm infants who were hospitalized from June2016 to June 2018 were randomly divided into observation group and control group, with 43 cases in each group. In the control group, the preterm infants were given routine nutritional support according to the energy required for daily weight. The preterm infants in the observation group were treated with early nutritional support strategy based on this treatment. Both groups were treated until the corrected gestational age for 40 weeks. The weight gain, birth weight recovery time and complications of preterm infants between two groups were observed, and the differences in neonatal behavioral neurological assessment(NBNA) between the two groups were compared at 40 weeks of corrected gestational age. Results The birth weight recovery time, nutritional support time and hospitalization time of the preterm infants in the observation group were significantly lower than those in the control group(P<0.05). The weight gain of the preterm infants in the observation group after birth weight recovery was significantly higher than that of the control group(P<0.05).The each item score and total scores of NBNA test in premature infants in the observation group were significantly higher than those in the control group(P<0.05). The incidence of complications in the preterm infants in the observation group was significantly lower than that in the control group(χ2=4.02, P<0.05). Conclusion The early nutritional support strategy can increase the neurobehavioral development and physical development rate of premature infants, shorten the time of nutrition support and hospitalization, and reduce the incidence of complications.
