硫酸镁对妊娠晚期羊水过少孕妇胎盘和胎膜水通道蛋白8及妊娠结局的影响

【摘要】 目的探讨硫酸镁对妊娠晚期羊水过少孕妇胎盘和胎膜水通道蛋白8(Aquaporin 8,AQP8)及妊娠结局的影响。方法选择2018年2~11月我院住院分娩妊娠晚期羊水过少孕妇72例,根据住院时间分为试验组(2018年7~11月) 38例和对照组(2018年2~6月) 34例。对照组给予葡萄糖、维生素C、丹参注射液静脉滴注,试验组给予葡萄糖、维生素、丹参注射液、硫酸镁注射液静脉滴注。治疗5 d后,比较两组孕妇临床症状、AQP8 mRNA表达及妊娠结局。结果试验组妊娠晚期羊水过少孕妇羊水指数(amniotic fluid dindex,AFI)明显高于对照组,胎儿脐动脉收缩期峰值流速与舒张期流速比值(S/D)明显低于对照组(P<0.05);羊膜上皮细胞、胎盘合体滋养细胞APQ8 mRNA明显高于对照组(P<0.05);胎儿窘迫等新生儿并发症发生率明显低于对照组(P<0.05)。结论硫酸镁有助于改善妊娠晚期羊水过少孕妇临床症状,降低新生儿并发症的发生,可能与促进羊膜上皮细胞、胎盘合体滋养细胞APQ8 mRNA表达有关。

【Abstract】 Objective To investigate the effect of magnesium sulphate on placenta,membranes aquaporin 8( AQP8) and pregnancy outcome of pregnant women with oligohydramnios.Methods Seventy-two pregnant women with oligohydramnios were divided into experimental group( July 2018 to November 2018,n = 38) and control group( February 2018 to June 2018,n = 34). The control group was given intravenous injection of glucose,vitamin C and danshen injection while the experimental group was given intravenous injection glucose,vitamins,danshen injection and magnesium sulfate injection. After 5 days of treatment,the clinical symptoms and AQP8 mRNA expression were compared between the two groups.The pregnancy outcomes were followed up and analyzed. Results The amniotic fluid index( AFI) in the experiment group were significantly higher than that in the control group while ratio of peak systolic to diastolic velocity of fetal umbilical artery( S/D) were significantly lower than that in the control group( P<0. 05).The expression of APQ8 mRNA in amniotic epithelial cells and placental syncytiotrophoblast cells in the experimental group was significantly higher than that in the control group( P<0. 05).Incidence of neonatal complications such as fetal distress in the experimental group was significantly lower than that in the control group( P<0. 05).Conclusion s Magnesium sulphate can help improve the clinical symptoms of pregnant women with oligohyniotic,and reduce the neonatal complications.This may be related to the promotion of APQ8 mRNA expression in amniotic epithelial cells and placental syncytiotrophoblast cells.
