硬性内窥镜故障分析及质量控制

【摘要】 目的:收集分析医院硬性内窥镜的故障数据,制定并实施质量控制措施,降低硬性内窥镜故障量。方法:剖析硬性内窥镜的结构与成像原理,通过目测法和机器检测法相结合,分析检测出的硬性内窥镜成像5种常见故障原因及解决对策,应用鱼骨图分析法分析硬性内窥镜故障的根本原因,从采购论证环节、操作回收环节和清洗消毒环节制定质量控制改进措施。结果:硬性内窥镜内部柱状晶体是最易破损的部件,实施质量控制改进措施后,由2017年出现9把故障硬性内窥镜降至2018年的3把,低于北京市16家医院2018年故障硬性内窥镜的平均值(5.375把)。结论:加强硬性内窥镜的质量检测和控制,能有效地降低其故障数量和临床手术风险,具有一定的临床应用价值。

【Abstract】 Objective: To collect and analyze the fault data of rigid endoscope in hospital, and drew up and implemented the measures of quality control so as to reduce the faulted quantity of rigid endoscope. Methods: The structure and imaging principle of rigid endoscope were dissected. Through combining the eye survey and the machine test to analyze the five kinds of common failure causes and countermeasure of the imaging of rigid endoscopes. The fishbone diagram was applied to analyze the root cause of the fault of rigid endoscope, and the improvement measure of quality control was formulated from three links included purchase and demonstration, operation and recycle, and cleaning and disinfection. Results: According to the data analysis, the interiorly columnar crystal was the most fragile part of rigid endoscope. After the improvements were implemented, the number of rigid endoscope with fault was reduced from 9 in 2017 year to 3 in 2018 year which was lower than the average number of rigid endoscope with fault(5.375) of 16 hospitals of Beijing in 2018 year. Conclusion: Strengthening the quality test and control can effectively reduce fault quantity and clinical risks of rigid endoscope, and it has a certainly clinical application value.
