围术期集束化护理在预防神经外科患者呼吸机相关性肺炎的研究
Study on the perioperative bundles of care in preventing VAP of patients of neurosurgery department
【摘要】 目的:分析围术期集束化护理降低神经外科手术后患者呼吸机相关性肺炎(VAP)发生率的效果,以减少术后患者VAP的发生。方法:选取在医院治疗的110例行神经外科全麻手术后需要机械通气的患者,按照数表法将其随机分为观察组和对照组,每组55例。对照组采取常规护理方法,观察组围术期采用集束化护理,比较两组患者VAP的发生率、病死率、机械通气时间以及入住重症监护室(ICU)时间。结果:观察组患者VAP的发生率和病死率均低于对照组,其差异有统计学意义(x~2=5.329,x~2=3.960;P<0.05);观察组患者机械通气时间、ICU入住时间明显低于对照组,其差异有统计学意义(t=2.282,t=3.352;P<0.05)。结论:围术期采用集束化护理能有效预防患者术后VAP的发生,缩短机械通气及ICU入住时间,降低VAP的病死率。
【Abstract】 Objective: To analyze the effect of the perioperative bundles of care in decreasing the incidence of ventilator-associated pneumonia(VAP) of patients post neurosurgery so as to reduce the occurrence of VAP of postoperative patients. Methods: A total of 110 patients who need mechanical ventilation after neurosurgery with general anesthesia were selected. They were divided into control group(n=55) and observation group(n=55) according to the number table method. The control group adopted conventional care and the observation group adopted the perioperative bundles of care method. And the incidence and mortality of VAP, the time of mechanical ventilation and the length of stay in intensive care unit(ICU) of two groups were compared. Results: The incidence and mortality of VAP of observation group were significantly lower than those of control group(x2=5.329, x2=3.960, P<0.05). And the time of mechanical ventilation and the length of stay in ICU of observation group were significantly lower than those of control group(t=2.282, t=3.352, P<0.05). Conclusion: The perioperative bundles of care can effectively prevent the occurrence of VAP in patients post-operation, and shorten the time of mechanical ventilation and length of stay in ICU, and decrease the mortality of VAP of patients.
