基于PLM评价模式下医疗设备维修费用的控制体系研究

【摘要】 目的:基于对医疗设备产品生命周期管理(PLM)数据的评价分析,探讨维修费用控制体系建设及其临床应用价值。方法:选取2016-2017年临床在用的5类798台医疗设备,按照维修管理模式的不同分为对照组和观察组。对照组396台医疗设备采用传统的经验维修管理模式;观察组402台医疗设备采用基于PLM数据评价的维修费用控制管理模式。对两组医疗设备使用相关数据(成本效益核算、故障频率、故障自修统计和升级报废)进行分析,评估医疗设备维修费用支出不合理问题,建立其购置、使用、维修和升级报废等行为管理体系。结果:两组设备价值与维修费用均呈正相关,其中对照组为高度相关(r=0.896,P<0.01),观察组为中度相关(r=0.732,P<0.01);两组间的医学影像设备、电动医疗设备、手术室设备、医用电子设备和急救设备年度维修费用占比比较,观察组均低于对照组,其差异有统计学意义(t=2.584,t=2.108,t=2.446,t=2.640,t=2.272;P<0.05);两组间的医疗设备故障频率分析显示,观察组的自修频率高于对照组,而人为故障频率低于对照组,两组对比差异有统计学意义(x~2=57.73,x~2=73.73;P<0.01)。结论:PLM评价模式下的维修费用控制体系能有效降低医疗设备维修费用,提高医学工程技术人员业务能力和医务人员的操作规范。

【Abstract】 Objective: To explore the construction and the value of clinical application of control system of maintenance cost based on the evaluation analysis of product lifecycle management(PLM) data of medical equipment. Methods: From 2016 year to 2017 year, 798 medical equipment of 5 categories were selected as the research objects. According to the different maintenance management modes, they were divided into control group and observation group. 396 medical equipment of control group adopted maintenance management mode with traditional experience, while 402 medical equipment of observation group adopted control management mode for maintenance cost based on PLM data evaluation. The correlative data(cost-effectiveness accounting, failure frequency, statistics of failure selfmaintenance, and upgrade and scrap) of the application of medical equipment of two groups were analyzed, and the unreasonable problem about the expenditure of maintenance cost of medical equipment was assessed. And then, the management system about purchase, use, maintenance, upgrade and scrapping and other behaviors was established. Results: There were positive correlation between equipment value and maintenance costs in both groups, and it was high correlation in control group(r=0.896, P<0.01), while observation group showed moderate correlation(r=0.732, P<0.01). And the ratios of the yearly maintenance costs of medical image equipment, electric medical equipment, equipment of operation room, medical electric equipment and emergency equipment of observation group were significantly lower than those of control group(t=2.584, t=2.108, t=2.446, t=2.640, t=2.272, P<0.05). The analysis of failure frequency of medical equipment between two groups indicated that the self-repair frequency of observation group was significantly higher than that of control group, while man-made failure frequency of observation group was significantly lower than that of control group(x~2=57.73, x~2=73.73, P<0.01). Conclusions: This control system of maintenance cost under PLM evaluation mode can effectively reduce the maintenance cost of medical equipment, and can improve the professional ability of medical engineering technicist and operation specification of medical personnel.
