【摘要】 目的分析甲状腺乳头状癌(papillary thyroid carcinoma,PTC)患者术前外周血血小板/淋巴细胞比值(platelet to lymphocyte ratio,PLR)和中性粒细胞/淋巴细胞比值(neutrophil to lymphocyte ratio,NLR)及其与预后的关系。方法将2012年7月至2014年11月中南大学湘雅二医院收治的60例PTC患者纳入PTC组,将该院同期63例健康体检者纳入健康组。检测并计算两组研究对象外周血PLR和NLR,分析PLR和NLR与PTC患者临床病理特征和预后的关系。采用Cox回归模型分析影响PTC患者预后的危险因素。结果 PTC组患者外周血PLR和NLR均显著高于健康组(均P<0.05)。PTC患者外周血PLR和NLR与TNM分期、淋巴结转移、多病灶均有关(均P<0.05)。外周血PLR和NLR低水平组患者术后死亡率均显著低于PLR和NLR高水平组(均P<0.05)。TNM分期、PLR、NLR均是影响PTC患者预后的独立危险因素(均P<0.05)。结论 PTC患者术前外周血PLR和NLR均升高,二者均可能影响PTC的发生和发展,有助于评估PTC患者预后。更多还原

【Abstract】 Objective To investigate the preoperative platelet to lymphocyte ratio (PLR) and neutrophil to lymphocyte ratio (NLR) of patients with papillary thyroid carcinoma (PTC) and their relationship with prognosis.Method From July 2012 to November 2014,60 patients with PTC in the Second Xiangya Hospital of Central South University were selected as PTC group,and 63 healthy people were selected as healthy group.The PLR and NLR in peripheral blood of the two groups were measured;the relationships between PLR,NLR and the clinicopathological characteristics,prognosis of PTC patients were analyzed;and the risk factors of prognosis of PTC patients were analyzed by Cox regression model.Result The PLR and NLR in peripheral blood of PTC group were significantly higher than those in healthy group (all P<0.05);the PLR and NLR in peripheral blood were related to TNM staging,lymph node metastasis and multifocality of PTC patients (all P<0.05);the postoperative death rate of patients in low PLR and NLR groups was lower than that in high PLR and NLR groups (all P<0.05).TNM staging,PLR and NLR were independent risk factors influencing the prognosis of PTC patients (all P<0.05).Conclusion The PLR and NLR in peripheral blood of patients with PTC were increased before operation,PLR and NLR may affect the occurrence and development of PTC,and both of them are helpful to evaluate the prognosis of PTC patients.更多还原
