损伤控制理论和四柱分型理论在胫骨平台骨折治疗中的临床研究
Clinical study of injury control theory and four column classification theory in the treatment of tibial plateau fracture
【摘要】 目的探讨损伤控制理论和四柱分型理论在胫骨平台骨折治疗中的应用效果。方法选取我院收治的60例胫骨平台骨折患者,依据损伤控制理论和四柱分型理论选择手术时机、手术方法和内固定方式,术后随访3年,统计治疗效果。结果术后患者的膝关节评分(88.32±3.58)分、膝关节活动度(132.46±17.25)°,高于术前的(60.49±5.07)分、(91.42±7.51)°(P<0.05);并发症发生率为6.67%,骨折愈合率为98.33%,平均愈合时间为(18.31±2.75)个月,全部患者下肢力线均恢复正常,双下肢长度差值为(0.63±0.17)cm。结论在胫骨平台骨折治疗中应用损伤控制理论和四柱分型理论可有效促进骨折愈合,提高膝关节活动度,改善患者膝关节功能,安全可靠,值得推广。
【Abstract】 Objective To explore the application effect of injury control theory and four column classification theory in the treatment of tibial plateau fracture. Methods 60 cases of tibial plateau fracture were selected. The operation time,operation method and internal fixation were selected by injury control theory and four column classification theory. The patients were followed up for 3 years.Results the knee joint score(88.32 ± 3.58),quality of life score(87.21±9.26),knee joint activity score(132.46 ±17.25)°were higher than those before operation(60.49±5.07),(62.29±11.43),(91.42±7.51)°P<0.05),complication rate(6.67%),fracture healing rate(98.33%),and average healing time(18.31±2.75)months The length difference of both legs was(0.63±0.17)cm.Conclusion the application of damage control theory and four column classification theory in the treatment of tibial plateau fracture can effectively promote the fracture healing,improve the knee joint activity,improve the knee joint function of patients,and is safe and reliable,which is worth popularizing.
