妊娠期糖尿病孕妇不同孕期肠道菌群的变化
Changes of gut microbiota in pregnant women with gestational diabetes mellitus over different pregnancy trimesters
【摘要】 目的探讨肠道菌群与妊娠期糖尿病(GDM)的相关性,并分析GDM孕妇不同孕期肠道菌群的变化情况。方法分别收集20例GDM组及对照组孕妇24周及37周的粪便,使用Illumina MiSeq PE300测序平台对肠道菌群的16S rRNA V3~V4区进行测序。结果 (1)GDM组孕妇的孕期增重明显小于对照组(P<0.05)。(2)门水平的相对分布情况:①组间比较:孕中期,2组孕妇差异无统计学意义,主要以厚壁菌门为主,其次为拟杆菌门,放线菌门,变形菌门。孕晚期,与对照组相比,GDM组拟杆菌门、放线菌门增加,厚壁菌门减少。②组内比较:GDM孕妇从孕中期到孕晚期厚壁菌门明显减低,拟杆菌门明显增加;对照组厚壁菌门稍减低,拟杆菌门稍增加。(3)OTU分析:孕中期,GDM组的特征肠道菌群为脱硫弧菌目、脱硫弧菌科、δ变形菌纲、丁酸弧菌属;对照组为瘤胃球菌属。(4)α多样性分析:①组间比较:孕中期,GDM组的α多样性稍低于对照组(P>0.05)。孕晚期,2组肠道菌群的α多样性差异无统计学意义。②组内比较:GDM组及对照组从孕中期至孕晚期α多样性均无明显变化。结论①随着孕期进展,GDM孕妇厚壁菌门比重减低,拟杆菌门比重增加。②GDM孕妇肠道菌群的α多样性低于对照组。③GDM孕妇孕期通过积极控制体质量,可能对改善肠道菌群α多样性有一定的积极作用。
【Abstract】 Objective To explore the correlation between gut microbiota and gestational diabetes mellitus(GDM), and to analyze the changes of gut microbiota in GDM pregnant women during different pregnancy trimesters.Methods The 24-week and 37-week stool samples were collected from 20 GDM pregnant women and control pregnant women, then subjected to sequencing of the 16 S rRNA V3-V4 regions of the gut microbiota by using the Illumina Mi Seq PE300 platform. Results(1)The weight gain during pregnancy significantly less in GDM women than in controls(P<0.05).(2)Phylum-based distributions of gut microbiota were at below: ①By between-group comparison,there was no significant difference in the phyla of the gut microbiota between the two groups of pregnant women during the second trimester, which was dominated by Firmicutes, followed by the Bacteroides, Actinomycetes, and Proteobacteria. During the third trimester, the GDM women showed increases in Bacteroides and Actinomycetes and decline in Firmicutes, compared with the control group. ②By within-group comparison, from the second through the third trimester of pregnancy, the GDM women showed significantly a reduction in Firmicutes and an increase in Bacteroides, while the control women showed slightly a reduction in Firmicutes and an increase in Bacteroides. ③Operational taxonomic units(OTU) based analysis: In the second trimester, the gut microbiota featured Desulfovibrio desulfuricans, δ Proteobacteria, and Vibrio butyricum in GDM women, in contrast to Rumenococcus the control group.(4)Alpha diversity analysis was at below: ①By between-group comparison, in the second trimester, the alpha diversity was slightly lower in the GDM group than that in the control group(P>0.05). In the third trimester, there was no significant difference in the alpha diversity of gut microbiota between the two groups. ②By within-group comparison: There was no significant change in alpha diversity in either the GDM group or the control group from the second through the third trimester. Conclusion ①Over the time of pregnancy, the GDM women show decreasing Firmicutes and increasing in Bacteroides in the components of gut microbiota. ②The alpha diversity of gut microbiota in GDM pregnant women is lower than that in the control group. ③Active weight control during pregnancy for GDM women may have a positive role in improving the alpha diversity of gut microbiota.
