FOCUS-PDCA模型在除颤仪应急调配中的应用

【摘要】 目的:探讨FOCUS-PDCA模型在院内除颤仪应急调配持续质量改进中的应用效果,缩短除颤仪平均调配时间。方法:按照F阶段(发现问题)—O阶段[成立持续质量改进(continuous quality improvement,CQI)小组]—C阶段(明确现行流程和规范)—U阶段(对发现的问题根本原因进行分析)—S阶段(选择流程改进方案)—P阶段(计划阶段)—D阶段(实施阶段)—C阶段(效果确认阶段)—A阶段(处理阶段)流程,根据除颤仪应急调配演练现况,采用头脑风暴法进行根本原因分析,通过制订全院除颤仪分布位点图、优化调配流程、开展专项培训等措施,改进应急调配现状,并对改善后除颤仪调配时间进行确认及总结分析。结果:除颤仪院内调配平均时间由7 min 25 s降低至3 min24 s,缩短了4 min 1 s。结论:在除颤仪院内应急调配改进工作中运用FOCUS-PDCA模型有效缩短了除颤仪应急调配时间,使除颤仪应急调配流程更加规范化、科学化,从而为临床提供更加优质的服务,保证医疗质量及安全。

【Abstract】 Objective To explore the application of the FOCUS-PDCA model in continuous quality improvement of defibrillator emergency deployment to shorten mean deployment time in the hospital. Methods Root cause analysis was executed with the investigation on defibrillator emergency deployment, brain storming method as well as the flow involving in Phase F(find a process to improve), Phase O(organize a continuous quality improvement(CQI) team), Phase C(clarify current knowledge of the process), Phase U(understand process variation and capability), Phase S(select a strategy for continued improvement), Phase P(plan), Phase D(do), Phase C(check) and Phase A(act). The current situation of emergency deployment was improved by formulating the distribution site map of defibrillator in the whole hospital, optimizing the deployment process, carrying out special training and other measures, and the deployment time of defibrillator after improvement was confirmed and summarized. Results The average time of hospital deployment of defibrillator was reduced from 7 min 25 s to3 min 24 s, with 4 min 1 s shortened. Conclusion The FOCUS-PDCA model is used in the emergency adjustment of the defibrillator to make the emergency deployment process standardized and scientific, which can effectively shorten the emergency deployment time of the defibrillator.
