肋骨接骨板内固定术与胸部护板外固定术治疗多发性肋骨骨折的效果比较
Comparison of effects of rib bone plate internal fixation and chrisofix external fixation in the treatment of multiple rib fractures
【摘要】 目的比较肋骨接骨板内固定术与胸部护板外固定术治疗多发性肋骨骨折的效果。方法选择2018年5月至2019年7月收治的58例多发性肋骨骨折患者为研究对象,根据治疗方法不同将其分为A组与B组,各29例。A组接受肋骨接骨板内固定术治疗,B组接受胸部护板外固定术治疗。比较两组的临床指标、疼痛程度、C反应蛋白(CRP)水平及并发症发生情况。结果 A组的呼吸频率恢复正常时间、血氧饱和度恢复正常时间、骨折愈合时间及住院时间均短于B组(P<0.05)。治疗后3 d、7 d、1个月,A组VAS评分均低于B组(P<0.05)。治疗后3 d,A组的CRP水平高于B组(P<0.05);而治疗后7 d、1个月,A组CRP水平明显低于B组(P<0.05)。B组并发症总发生率高于A组(P<0.05)。结论肋骨接骨板内固定术治疗多发性肋骨骨折的效果优于胸部护板外固定术,可促进术后骨折端愈合与呼吸状态恢复,减轻术后疼痛程度及应激反应,也能减少并发症发生,可作为首选手术方案。
【Abstract】 Objective To compare the effects of rib bone plate internal fixation and chrisofix external fixation in the treatment of multiple rib fractures. Methods A total of 58 patients with multiple rib fractures who were treated from May2018 to July 2019 were selected as the study objects, and the patients were divided into group A and group B according to different treatment methods, with 29 cases in each group. The group A received rib bone plate internal fixation, and the group B received chrisofix external fixation. The clinical indexes, the degree of pain, level of C-reactive protein(CRP) and complications were compared between the two groups. Results The recovery normal time of respiratory rate and oxygen saturation, fracture healing time and hospital stay time in the group A were shorter than those in the group B( P<0.05).At 3 d, 7 d and 1 month after treatment, VAS scores in the group A were lower than those in the group B(P<0.05). At 3 d after treatment, CRP level in the group A was higher than that in the group B(P<0.05); but at 7 days and 1 month after treatment, the CRP level in the group A were significantly lower than those in the group B(P<0.05). The total incidence of complications in the group B was higher than that in the group A(P<0.05). Conclusion Rib bone plate internal fixation in the treatment of multiple rib fractures is more effective than chrisofix external fixation, it can promote postoperative fracture end healing and respiratory state recovery, reduce postoperative pain degree and stress response, and also reduce the occurrence of complications, which can be used as the first choice of surgical scheme.
