【摘要】 目的分析不同类型人群血清中肝纤维四项检测结果,探讨其在肝纤维化临床诊疗中的应用价值。方法收集本院227例健康体检者(健康体检组)和267例不同类型肝病患者(肝病患者组)的血清,运用上转发光免疫分析技术,进行肝纤维四项即透明质酸(HA)、Ⅲ型前胶原(PC-Ⅲ)、Ⅳ型胶原(Ⅳ-C)、层粘连蛋白(LN)水平检测。结果健康体检组HA、PC-Ⅲ、Ⅳ-C、LN水平均处于该检测试剂阴性结果范围。肝硬化组和原发性肝癌组HA、PC-Ⅲ、Ⅳ-C、LN阳性检出率显著高于慢性乙肝组和慢性丙肝组(P<0.05)。肝病患者组的HA、PC-Ⅲ、Ⅳ-C、LN水平均处于该检测试剂阳性结果范围,不同类型肝病患者组HA、PC-Ⅲ、Ⅳ-C、LN水平均高于健康体检组(P<0.05);肝硬化组和原发性肝癌组的HA、PC-Ⅲ、Ⅳ-C、LN水平均高于慢性乙肝组和慢性丙肝组(P<0.05)。结论不同类型人群血清中肝纤维四项检测结果能够反映其肝纤维化风险程度,临床实践中可以考虑将肝纤维四项检测纳入健康体检,从而更早地提示肝脏损伤状况,为临床诊疗提供科学依据。更多还原

【Abstract】 Objective To analyze the results of four tests of liver fiber in serum of different types of people, and to explore its application value in the clinical diagnosis and treatment of liver fibrosis. Methods The serum of 227 healthy physical examination people(healthy physical examination group) and 267 patients with different types of liver diseases(liver disease patients group) were collected, the levels of the four tests of liver fiber such as hyaluronic acid(HA),procollagen Ⅲ(PC-Ⅲ), collagen Ⅳ(Ⅳ-C) and laminin(LN) in liver fibers were measured by upconversion immunoassay.Results The levels of HA, PC-Ⅲ, Ⅳ-C and LN in the healthy physical examination group were in the negative range of the test reagent. The positive rates of HA, PC-Ⅲ, Ⅳ-C and LN in the cirrhosis group and the primary liver cancer group were significantly higher than those in the chronic hepatitis B group and the chronic hepatitis C group(P<0.05). The levels of HA, PC-Ⅲ, Ⅳ-C and LN in the liver disease patients group were in the positive range of the test reagent, the levels of HA, PC-Ⅲ, Ⅳ-C and LN in the different types of liver disease groups were higher than those in the health physical examination group(P<0.05); the levels of HA, PC-Ⅲ, Ⅳ-C and LN in the cirrhosis group and the primary liver cancer group were higher than those in the chronic hepatitis B group and the chronic hepatitis C group(P<0.05). Conclusion The results of four tests of liver fiber in serum of different types of people can reflect the risk degree of liver fibrosis. In clinical practice, four tests of liver fiber can be considered to be included in the physical examination, so as to indicate the status of liver injury earlier and provide scientific basis for clinical diagnosis and treatment.更多还原
