血栓弹力图在创伤性大出血患者急救输血中的价值
Value of thromboelastography in emergency transfusion of patients with traumatic hemorrhage
【摘要】 目的探讨血栓弹力图(TEG)在指导创伤性大出血患者急救输血中的价值。方法选取本院2016年1月至2017年6月收治的40例创伤性大出血行常规输血治疗的患者作为对照组,另选取2017年7月至2019年2月收治的42例TEG指导用血的患者为研究组。比较两组患者输血效果。结果输血后,两组患者的PT、APTT、TT均缩短,FIB、PLT均升高,且研究组均优于对照组(P<0.05);输血后,研究组患者的R、K均短于输血前,α角大于输血前,MA高于输血前,LY30低于输血前(P<0.05);研究组患者的红细胞、血浆用量明显低于对照组,24 h好转率高于对照组,渗血时间和住院天数短于对照组(P<0.05)。结论 TEG用于指导创伤性大出血患者的输血可以有效改善患者的凝血功能,缩短患者的住院时间,减少血液制品的使用量,具有较高的临床应用价值。
【Abstract】 Objective To explore the value of thromboelastography(TEG) in guiding emergency transfusion in patients with traumatic hemorrhage. Methods Forty patients with traumatic hemorrhage treated with routine blood transfusion from January 2016 to June 2017 were selected as control group, and 42 patients who treated with TEG guided blood transfusion from July2017 to February 2019 were selected as study group. The effects of blood transfusion between the two groups were compared.Results After blood transfusion, PT, APTT and TT of the two groups shortened, FIB and PLT increased, and those in the study group were better than the control group(P<0.05); after blood transfusion, the R and K of patients in the study group were shorter than those before blood transfusion, the α angle was larger than that before blood transfusion, MA was higher than that before blood transfusion, the LY30 was lower than that before blood transfusion(P<0.05); the dosage of RBC and plasma in the study group were significantly lower than those in the control group, the 24-hour improvement rate was higher than that in the control group, and the time of bleeding and hospitalization were shorter than those in the control group(P<0.05). Conclusion TEG in the guidance of blood transfusion in patients with traumatic hemorrhage can effectively improve the patients’ blood coagulation status, shorten the hospital stay of patients and reduce the use of blood products, which has higher clinical application value.
